Clinical and Serological Predictors of Remission in Rheumatoid Arthritis Are Dependent on Treatment Regimen

Abstract: Early intensive regimens have become the gold standard in the treatment of early RA. Our study suggests that this intensive approach is only superior to monotherapy in certain subsets of patients. Although these are unlikely to be the only predictors of treatment response, our study brings us a step closer to achieving personalized medicine in RA.

“…Because the original aim of the CARDERA studies was to investigate the performance of combination therapy with reference to monotherapy, a placebo group was not assigned. Rheumatoid factor (RF), a biomarker providing clinical information on the antibody composition of patient serum, was assayed as described previously (16). Follow-up was 2 years.…”

“…Around 10% of the patients had very high titre values (Table 1). 3 C-reactive protein is an acute phase reactant that is widely used and a rise in its concentration is correlated with disease activity. CRP is a better marker than ESR which could identify the inflammatory burden and it closely overlaps with TJC, SJC and pain scale (VAS).…”

“…Although some clinical and serologic factors have been shown to be associated with response to certain treatment options 12, 13, there have yet been no consistent predictors that could identify an individual patient with a higher chance of responding to a particular therapy compared with another 14, 15.…”
