Background: Patients with narrow alveolar ridges requiring dental implants may be managed by residual ridge augmentation or guided bone regeneration procedures followed by the placement of conventional diameter implants. All these procedures need more time, more cost, and more clinician experience. Mini implants are an alternative option in case of the reduced residual alveolar ridge. Aim: The aim of this study was to evaluate the crestal bone changes, survival rates, and patient satisfaction with mini dental implant. Methodology: An electronic searching was performed in the following databases, Pubmed (1990 to 25 October 2016 and Cochran (1999Cochran ( to 2016. In addition, hand searching was performed in implant-related journals and through the references of included studies. Result: Four articles resulted after the final filtration, from which three studies were randomized controlled trials and one non-randomized controlled trial. The follow-up period was extended at least 1 year in all studies. The results of the included studies reveal that the marginal bone loss, survival rate, and patient satisfaction of the mini implant were comparable to those resulting from the conventional implants. Conclusion: No significant difference was observed between the mini implant and the conventional implants regarding the marginal bone loss, implant survival rate, and patient satisfaction. However, this review depends on few numbers of studies that accompanied with a high degree of bias, so more evidence to validate this treatment is still necessary. Clinical Significance: The flapless technique of the small implant diameter offers the advantage of less invasion and less trauma to the tissue. Furthermore, the mini implants approach makes promise solution for those patients with medical and/or financial problems to use conventional implants so that they increase the clinical applications of dental implants.

This in vitro study aimed to use failure stress and implant abutment interface (IAI) microgap size to find the compromised axial angle range of angulated zirconia abutments with a titanium base in narrow diameter implants in the esthetic region. A three-dimensional (3D) finite element model of maxillary central incisor implant prosthesis was reconstructed. Angulated zirconia abutments (0 , 15 , 30 , and À 15 ) with a titanium base in narrow diameter implants (3.3 Â 12 mm, Bone level, Roxolid SLActive, Straumann AG, Switzerland) were designed to simulate clinical scenarios of buccal inclination 0 , 15 , and 30 , and palatal inclination 15 of the implant long axis. Straight titanium abutment and pure titanium implant were used as two control groups. An oblique force at 30 inclination to the long axis of the crown was applied 3 mm below the incisal edge on the palatal surface of the prosthesis.Under simulated dynamic chewing force, the stress distribution of the implant components and surrounding bone were investigated. The relative micromotion displacement between the implant and abutment models at the IAI area was recorded, and the influence of tightening torque on the IAI microgap was evaluated. The angulation of the zirconia abutment could affect the stress value and IAI microgap of implant restorations. When the zirconia abutment angle increased from À15 to 30 , the stress on the central screw, titanium base, and surrounding bone tissue gradually increased by 9%, 20%, and 23%, respectively. The stress levels of the 30 zirconia abutment group showed the risk of exceeding the threshold. When the long axis of the implant was inclined in the palatal direction, the À15 angle abutment reduced the stress by 3% and reduced the strain level of the implant system by 17% and the surrounding bone tissue by 26%. Under simulated dynamic chewing load, the displacement between the implants and the abutment occurred in each group of the implant system, and the amplitude of the micromotion fluctuated with the change in Yuqiang Zhang and Ping Yu contributed equally to this article and should be regarded as co-first authors.

Objectives Stability values of mini-implants (MIs) are ambiguous. Survival data for MIs as supplementary abutments in reduced dentitions are not available. The aim of this explorative research was to estimate the 3-year stability and survival of strategic MIs after immediate and delayed loading by existing removable partial dentures (RPDs). Material and methods In a university and three dental practices, patients with unfavorable tooth distributions received supplementary MIs with diameters of 1.8, 2.1, and 2.4 mm. The participants were randomly allocated to group A (if the insertion torque ≥ 35 Ncm: immediate loading by housings; otherwise, immediate loading by RPD soft relining was performed) or delayed loading group B. Periotest values (PTVs) and resonance frequency analysis (RFA) values were longitudinally compared using mixed models. Results A total of 112 maxillary and 120 mandibular MIs were placed under 79 RPDs (31 maxillae). The 1st and 3rd quartile of the PTVs ranged between 1.7 and 7.8, and the RFA values ranged between 30 and 46 with nonrelevant group differences. The 3-year survival rates were 92% in group A versus 95% in group B and 99% in the mandible (one failure) versus 87% in the maxilla (eleven failures among four participants). Conclusions Within the limitations of explorative analyses, there were no relevant differences between immediate and delayed loading regarding survival or stability of strategic MIs. Clinical relevance The stability values for MIs are lower than for conventional implants. The MI failure rate in the maxilla is higher than in the mandible with cluster failure participants.
