Clinical and radiobiological consideration of cyclical hypofractionated radiation therapy also known as QUAD Shot for neglected skin cancer disfiguring the face of a non-compliant patient who was refusing surgery and protracted radiation therapy: case report

Abstract: Although surgery is the mainstay of local treatment for skin cancer, definitive radiation therapy (RT) has been also applied for patients who are unable to tolerate surgery. Definitive RT regimens usually consist of daily treatment for 4–7 weeks. Such protracted daily RT regimens, however, would not be feasible for non-compliant patients or patients who are unable to make multiple daily trips for weeks. Without treatment, however, skin cancers can continuously progress and cause distressing symptoms. A cyclica… Show more

“…Radiation was administered to 50 Gy in 4–5 fractions 17 . A study calculating the biological effectiveness of the quad shot regimen found 4–5 cycles is comparable to that from protracted definitive RT regimens 18 . Thus, in the palliative setting, quad shot is especially favorable as it meets all the major goals of therapy: attaining local tumor control, providing symptom palliation, an abbreviated treatment course, and minimal toxicity.…”

“…For other organs at risk, the constraints were as low as reasonably achievable and followed suggestions from published recommendations. 8 For subsequent quad shots, patients underwent new CT-simulation scans to revise PTVs accounting for tumor response from previous quad shot.…”

“…Radiobiologic effectiveness of 3 or more cycles of quad shot is reportedly comparable to that of protracted definitive RT regimens, and hypofractionation may provide particular benefit to radio-resistant low alpha-beta ratio cancers. 7 , 8 , 9 , 10 Herein, we report responses from palliative RT using quad shot in elderly or frail patients with nonosseous metastatic/recurrent cancer in various anatomic sites with varying histology.…”

“…IMRT-QUAD was recommended for elderly patients comorbid with head and neck cancer (including parotid gland cancer) in recent studies, which was reasonable and safe to apply for symptom palliation with a reduced number of fractions. IMRT-QUAD can produce more rapid regression in tumors with an earlier alleviation of malignancy-related symptoms, and minimize acute and late toxicities in normal organs ( 30 , 31 ), impacting positively on patients’ quality of life (QoL) ( 32 ). Adjustment of PORT fractionation may need for patients in this high-risk group.…”
