Mitochondrial DNA Mutation Analysis in Breast Cancer: Shifting From Germline Heteroplasmy Toward Homoplasmy in Tumors

Studies have suggested a potential role of somatic mitochondrial mutations in cancer development. To analyze the landscape of somatic mitochondrial mutation in breast cancer and to determine whether mitochondrial DNA (mtDNA) mutational burden is correlated with overall survival (OS), we sequenced whole mtDNA from 92 matchedpaired primary breast tumors and peripheral blood. A total of 324 germline variants and 173 somatic mutations were found in the tumors. The most common germline allele was 663G (12S), showing lower heteroplasmy levels in peripheral blood lymphocytes than in their matched tumors, even reaching homoplasmic status in several cases. The heteroplasmy load was higher in tumors than in their paired normal tissues. Somatic mtDNA mutations were found in 73.9% of breast tumors; 59% of these mutations were located in the coding region (66.7% non-synonymous and 33.3% synonymous). Although the CO1 gene presented the highest number of mutations, tRNA genes (T, C, and W), rRNA 12S, and CO1 and ATP6 exhibited the highest mutation rates. No specific mtDNA mutational profile was associated with molecular subtypes of breast cancer, and we found no correlation between mtDNA mutational burden and OS. Future investigations will provide insight into the molecular mechanisms through which mtDNA mutations and heteroplasmy shifting contribute to breast cancer development.

Resumen El cáncer de mama localmente avanzado continúa siendo un problema importante en los países en desarrollo y una presentación común en todo el mundo. En nuestro país, el 45% de los casos de cáncer de mama son diagnosticados en estadios localmente avanzados. La cobertura cutánea de la pared torácica después de una resección extensa en pacientes con cáncer de mama siempre ha representado un reto para el grupo quirúrgico, por lo que el colgajo de dorsal ancho se usa ampliamente en la reconstrucción de la misma. Usualmente, con la técnica clásica, el tamaño de la isla de piel no es mayor a 10 cm. Por esta razón, en nuestro instituto se ha implementado el colgajo de dorsal ancho extendido con la finalidad de cubrir grandes defectos de hasta 40 cm. Se realizó un estudio retrospectivo de todos los casos de dorsal ancho extendido durante un periodo de cinco años. Un total de 30 pacientes fueron reconstruidas con un colgajo de dorsal ancho, de las cuales 15 fueron tratadas con un colgajo extendido, y se lograron resecciones tumorales de hasta 30 cm. La edad promedio fue de 42.6 años. La dimensión del tumor registrada fue de hasta 30 cm. Hubo una tasa baja de complicaciones que requirieron reintervención quirúrgica (6.6%) en un seguimiento promedio de 15.8 meses. Esta técnica es reproducible, se realiza en un solo tiempo quirúrgico y puede ser considerada para la reconstrucción parcial o total de grandes defectos estéticos, puramente paliativos o curativos. El éxito de esta técnica depende de la selección adecuada de las pacientes y de un tratamiento multidisciplinario coordinado entre los cirujanos de mama y el cirujano plástico. BackgroundAttendance to mammography screening is low in Mexico with late breast cancer (BCa) intervention. This study investigates reasons for low attendance in screening programs. Socioeconomic and cultural factors were investigated. The present national BCa screening programs are discussed. MethodsThis is a cross-sectional study involving 1066 women in the Monterrey Metropolitan area. Women aged 40-79 years old with low-income/middle-income completed a questionnaire on socioeconomic and cultural factors. A descriptive and multivariate analysis was done.ResultsIn total, 27 percent had never attended mammography and about 61 percent had only done a mammography during the last 2 years. Sixty-seven percent of the women went to the doctor only when they "felt sick". The two principal reasons for not attending were financial (38%) and fatalism related to religious faith (80%). ConclusionsThis study, is the first to quantify both the socioeconomic and cultural factors in women in Mexico. It analyzes the relationship of the factors precluding BCa screening, seeking care for breast symptoms and subsequent treatment. LI and MI women belong to different socioeconomic and cultural subgroups in MMa. Affordable National state of the art prevention programs are needed, considering a heterogeneous population where the majority have low income/are poor. Tailored information on the importance of mammography attendance is needed where the Catholic Church could help to deliver information. Subsequent BCa treatment should be based on current guidelines.
