The invasive and proteolytic activities of P. gingivalis clinical isolates have been shown to be associated with heterogenic virulence, as determined in a mouse abscess model. The aims of the present study were to identify a P. gingivalis strain with a low virulence among clinical isolates, based on its invasive ability and cytokine proteolytic activities, and to explore the preferential degradation of a certain cytokine by P. gingivalis. P. gingivalis ATCC 33277, W50, and 10 clinical isolates were used. After incubating bacteria with IL-4, IL-6, IL-10, IL-17A, TNFa, IFNg, and IL-1a, the amounts of remaining cytokines were determined by ELISA. Invasion ability was measured by a flow cytometric invasion assay. There was inter-strain variability both in the cytokine proteolytic activities and invasion ability. In addition, differential degradation of cytokines by P. gingivalis was observed: while IFNg and IL-17A were almost completely degraded, inflammatory cytokines TNFa and IL-1a were less susceptible to degradation. Interestingly, the invasion index, but not cytokine proteolytic activities, of P. gingivalis had strong positive correlations with clinical parameters of subjects who harbored the isolates. Therefore, the invasive ability of P. gingivalis is an important virulence factor, and the bacterial invasion step may be a good target for new therapeutics of periodontitis.

Objetivo: revisar a literatura sobre o tratamento das doenças periodontais associado a probióticos, identificando as funcionalidades e os principais agentes microbianos empregados. Material e método: foi realizada busca nas bases de dados eletrônicas PubMed e Science Direct, empregando os termos de busca probiotics e periodontal diseases. Como critérios de inclusão, foram selecionadas pesquisas clínicas originais e ensaios clí-nicos publicados em português ou inglês. Resultados: após pesquisa e minuciosa revisão por título e resumo de cada estudo, 40 ensaios clínicos randomizados foram selecionados para avaliação dos desfechos observados. Todos os estudos empregaram probióticos associados a raspagem, alisamento e polimento coronorradicular. A cepa bacteriana mais utilizada é o Lactobacillus reuteri. Foi demonstrado que os probióticos conferem potencial auxilio ao tratamento das lesões periodontais. Embora os parâmetros avaliados nem sempre sejam beneficiados pelo tratamento, o uso dos microrganismos benéficos reduziu a necessidade de intervenção cirúr-gica principalmente em pacientes com bolsas profundas. Considerações finais: A utilização dos probióticos se mostra segura e traz pequenos ganhos auxiliares no tratamento das doenças que acometem o periodonto. Desenhos de estudos com rigor metodológico e amostras representativas são encorajados visando analisar e testar os benefícios desses agentes. IntroduçãoOs probióticos são definidos como bactérias que promovem benefícios, portanto necessitam promover efeitos fisiológicos, ser seguros para o uso, está-veis ao ácido gástrico e à bile, além de aderir à mucosa intestinal 1 . A Organização Mundial da Saúde (OMS) define probióticos como microrganismos vivos que, administrados em quantidades adequadas, são capazes de promover benefícios ao hospedeiro a partir da supressão de patógenos endógenos e exó-genos, beneficiando a resposta imune 2 . Existem inúmeras células bacterianas que habitam o organismo dos seres humanos. Estudos recentes buscam o mapeamento desse microbioma, para, de forma unificada e específica, centrar ações terapêuticas cada vez mais na especificidade das lesões e doenças, evitando resistência microbiana, sobretratamentos, superdoses medicamentosas e utilização de medicamentos pouco ativos ou inativos para dada patologia 3 . Os probióticos estão no centro dessas descobertas, sendo os principais vetores para promoção de ação nos diferentes microrganismos endógenos humanos, visando ao desenvolvimento do processo de saúde no hospedeiro 4,5 . Probiotics are defined as live microorganisms that, when administered in adequate amounts, confer a health benefit on the host. Probiotics have been used to directly modify the resident oral microbiome and proposed to modulate immune responses. In dentistry, probiotics have been employed as useful adjuncts for the reduction of caries development, suppressing oral Candida infection and controlling halitosis. Plaque-induced gingivitis is a gingival inflammation caused by the adherent bacterial biofilm around teeth. Gingivitis and periodontitis are considered to be a continuum of the same inflammatory process, although many gingivitis lesions do not progress to periodontitis. Periodontitis is an inflammatory process that affects the attachment structures of teeth. It constitutes a second cause of tooth loss worldwide. Conventional treatment modalities of periodontal disease include non-surgical and/or surgical management, with an emphasis on mechanical debridement. However, mechanical debridement as a sole therapy is not always effective to improve clinical parameters. A growing number of studies support probiotic therapy to prevent or treat gingivitis and periodontitis. Oral administration of probiotics is an effective adjunct in reducing pathogenic bacteria and improving clinical signs of disease. Probiotics may serve as adjunct or replacement therapy substitute antibiotics in managing human periodontal infections in future.
