The aim of this study was to evaluate, based on the analysis of neurobiochemical markers, the effectiveness of pathogenetically substantiated therapy for disorders of the psychomotor and physical development of children in the first year of life who underwent perinatal hypoxia. Materials and Methods:The study included 419 patients (52% boys and 48% girls) aged from 1 to 6 months. The main group included 336 patients in the first year of life who received inpatient treatment for perinatal CNS damage of different degrees of severity. The main group was divided into two subgroups according to age: Group 1 (n=163) between the ages of 1 and 3 months and Group 2 (n=173) between the ages of 4 and 6 months. In accordance with the severity of the CNS lesion, the main group was also divided into 3 subgroups: mild degree (n=122), moderate degree (n=118), and severe degree (n=96). The control group included 83 apparently healthy children (n=43 between the ages of 1 to 3 months and n=40 between the ages of 4 to 6 months). The analysis of individual physical development of the children was carried out using Z scores (weight, age, head circumference) and centiles (7 intervals ("corridors")) according to the WHO standard program WHO AnthroPlus, The concentrations of biochemical markers (L-Homocysteine, beta-NGF, S100 protein, angiotensin II) in the blood were evaluated in all children at admission, as a routine entry investigation. In accordance with a treatment regimen, the main group was also divided into 2 subgroups: subgroup A (n=170), patients who received therapy depending on a general clinical manifestation; and subgroup B (n=166), patients who received therapy depending on a dominant syndrome and variability of neurobiochemical markers.Results: We found that Scheme B showed advantages for all studied neurobiochemical markers, with statistical significance for L-Hcy regardless of the age group. The positive dynamics were found in the ND severity against the background of Scheme B regardless of the age group and the degree of severity of the CNS lesion. Thus, the pronounced positive dynamics in the levels of neurotrophic and neurovascular markers of the CNS lesion in all age groups reflects the advantage of pathogenetic therapy. Objective: proteomic analysis of blood serum of children with bronchial asthma with a history of indication of the pathology of the perinatal period.Materials and methods: a comprehensive examination of children suffering from asthma and patients of the control group using proteomic analysis of blood serum was carried out. Proteomic analysis of blood serum was performed using two-dimensional high-resolution gel electrophoresis followed by staining of gels with fl uorescent dye and time-of-fl ight MALDI mass spectrometry. The proteins were identifi ed using Mascot MS peptide fi ngerprint analysis algorithm and Swiss-Prot protein database.Results: comparative analysis of the proteomic profi le of blood serum of children with pathology of the central nervous system in the perinatal period and developed in the subsequent bronchial asthma and control group patients allowed to establish that number proteins products were reduced in this disease. The proteins in the molecular mass range 16-33 kDa (p<0.05) were identifi ed: glutathione peroxidase 3, transthyretin and system components compliment С4В and C3.Conclusion: modifi cation of the production of these proteins indicates the presence of damage at the molecular level, leading to metabolic and functional disorders in the body of a patient with asthma having a history of indicating the pathology of the perinatal period.

Hypoxic-ischemic brain damage of the fetus and newborn with hemodynamic disorders in the “mother-placenta-fetus” system

Objective: to identify the dependence of the severity of cerebral disorders in newborns and children of the first year of life on the indicators of blood flow in the utero-placental complex. Materials and methods: a total of 184 full-term newborns were examined in the period from birth to one year of life. The main group included children with cerebral ischemia of II and III severity and its consequences (group II, n=78; group III, n=42). Group I included newborns without signs of central nervous system damage, 14 of them had neurological symptoms by the end of the neonatal period (after a month, group I n =50, group II n =92). All children underwent general clinical examination, assessment of neurological status, ultrasound examination of the brain, transcranial dopplerography of cerebral vessels, electroencephalography. Maternal medical records were analyzed to identify hemodynamic parameters in the "mother-placenta-fetus" system at 12-13, 20-21, 28-32, and 36-40 weeks of gestation. Results: The features of uterine and fetal blood flow in the dynamics of gestation in mothers of the examined groups of children were determined. The associations between the values of Pi AUD, AUS, and AUM in the second and third trimesters of pregnancy in different groups were shown. The relationship between disorders of uteroplacental hemodynamics and the severity of cerebral pathology was revealed. A "method of antenatal prediction of the severity of cerebral disorders in newborns" was proposed. Conclusions: The obtained results make it possible to predict the risk of hypoxic-ischemic damage to the central nervous system in newborns antenatally using hemodynamic parameters in the mother-placenta-fetus system at 36 weeks of gestation.
