Clearance of pediatric actinic prurigo with dupilumab

Abstract: Actinic prurigo is a rare, idiopathic chronic photodermatosis of childhood characterized by excoriated papules, nodules, and plaques in sun-exposed areas. It is notoriously difficult to treat. The disorder involves a type IV hypersensitivity reaction driven by both Th1 and Th2 inflammatory pathways, the latter of which leads to secretion of | 1177

“…Chronic photodermatoses including chronic actinic dermatosis (CAD) and actinic prurigo are induced by exposure to UV-radiation and can resemble photosensitive AD. Reports showed efficacy of dupilumab in 16 cases of CAD ( Table 8 ) [ 252 , 253 , 254 , 255 , 256 , 257 , 258 ] as well as in a pediatric patient with actinic prurigo [ 259 ], leading to higher minimal erythema doses in repeated UV-phototesting in some cases.…”

“…For severe AP, thalidomide and cyclosporine are classically used treatments, but patients frequently flare upon withdrawal thus making chronic use necessary. 1 AP patients usually require chronic therapy, and the potentially severe side effects particularly with chronic use of thalidomide and cyclosporine limit their utility, especially in the pediatric population. 1 , 3 Antimalarials and phototherapy with narrowband UV-B or psoralen plus UV-A have also been used to desensitize patients to UV activity.…”

“…Actinic prurigo (AP) is a rare chronic photodermatosis typically seen in indigenous populations of North, Central, and South America. 1 Approximately 93% of Latin American patients with AP are found to be positive for HLA-DR4, of which 80% have the DRB1∗0407 allele. 2 These individuals are predisposed to type IV hypersensitivity reactions to UV radiation.…”

“…Clinical remission and improved quality of life were observed following three courses of 300 mg monthly for 6 months and a fourth year-long course. Clearance of pediatric actinic prurigo and adult actinic dermatosis has also been reported with dupilumab, an interleukin-4 receptor antagonist that disrupts the Th2 immunologic pathway [10,11]. Adult patients with resistant chronic actinic dermatitis have also responded to tofacitinib therapy, an oral Janus kinase inhibitor [12].…”
