Clear Cell Myomelanocytic Tumor of the Urinary Bladder

Abstract: Clear cell myomelanocytic tumors are a recently described neoplastic growth considered to be a member of the family of perivascular epithelioid cell tumor. These tumors have a predilection for falciform ligament/ligamentum teres. We report an additional case arising from the muscularis propria of the urinary bladder in a 33-year-old woman. The tumor consisted of clear to eosinophilic, epithelioid, and spindled cells arranged in fascicles or packets. A delicate vascular stroma was found among the nests. Immunoh… Show more

“…The first report by Pan and colleagues described a 33-year-old Taiwanese woman who was found to have a PEComa of the bladder during a workup for dysmenoorhea. 8 The patient did not have any lower urinary tract symptoms or gross hematuria. She underwent partial cystectomy with total excision of the tumour and remained without evidence of disease with 6 years of follow-up.…”

“…6 PEComas have been documented in a variety of anatomical locations, including the pancreas, small and large intestine, ligamentum teres/falciform ligament, common bile duct, kidney, bladder, prostate, breast, uterus, cervix, vulva, ovary, broad ligament, heart, base of skull and soft tissue. 8,[12][13][14][15] The major differential diagnosis of this tumour around the anatomical site of the urinary bladder, includes smooth muscle tumours. Smooth muscle tumours, including leiomyoma and leiomyosarcoma, are the most common mesenchymal tumours of the urinary bladder in adults.…”

“…This lack of criteria is primarily a result of the tumour's rarity and the limited follow-up reported in most cases 12,17,18 . The limited number of malignant cases published thus far have described malignant spread as being characterized by a locally infiltrative growth pattern, vascular invasion, a high mitotic rate, necrosis, and tumour size exceeding 8 cm 4,14,16,[19][20][21][22] . The clinical behaviour of pecomas is generally benign.…”

“…The perivascular myoid cell concept has yielded the recognition of myopericytoma [3][4][5][6][7][8], myofibroma [4], myofibromatosis [9], angioleiomyoma [10], and glomus tumors (GTs) [9,11,12], while the perivascular epithelioid cell concept that shows melanocytic lineage has led to the recognition of PEComa [13,14], clear cell sugar tumor [15,16], angiomyolipoma [17], and lymphangiomyomatosis [1]. These tumors belong to a spectrum of perivascular myoid cell tumors and perivascular epithelioid cell tumors, and there are some overlaps among these entities.…”
