Clear cell fibrous papule: report of a case mimicking a balloon cell nevus

Abstract: Clear cell fibrous papule (FP) is a rare variant of FP. We report a 39-year-old female patient who presented with a dome-shaped papule on the nose. The diagnosis of clear cell FP was made based on histological and immunohistochemical studies. Interestingly, scattered S-100 cells were admixed within the lesion, a finding hitherto not reported. The S-100 positivity may be misleading and should be cautiously interpreted.

“…Twenty‐two cases of clear cell fibrous papule have been reported to date (including the granular cell variant) . Like the common angiofibroma, clear cell fibrous papule appears as a dome‐shaped skin‐colored to slightly erythematosus solitary papule, which appears most commonly on the nose of adults.…”

“…Hypercellular FP presents as marked fibroblastic proliferation and fibroplasia of spindle to round cells, mimicking atypical fibroxanthoma and melanoma 2 . Clear cell FP, posing as the most difficult variant for definitive histopathological diagnosis, reveals a proliferation of single or small clusters of cells with vacuolated clear cytoplasm, with dilated vessels and spindle/stellate cells in the background 14 . Epithelioid FP reveals large cells with abundant pink cytoplasm and without apparent granules, along with increased numbers of small blood vessels within and around the cellular component 15 .…”

“…Des variantes ont été décrites à partir des années 1990 sur la base de cas isolés ou de petites séries : la forme à cellules granuleuses [2], la forme à cellules claires [3][4][5][6] et la forme à cellules épithélioïdes [7]. Dans la seule étude de grande ampleur publiée à ce jour, les auteurs ont également identifié d'autres variantes : hypercellulaire, inflammatoire, pigmentée, et enfin à cellules pléomorphes [8].…”
