The increase in incidences of pediatric Crohn’s Disease (CD) worldwide has been strongly linked with dietary shifts towards a Westernized diet, ultimately leading to altered gut microbiota and disturbance in intestinal immunity and the metabolome. Multiple clinical studies in children with CD have demonstrated the high efficacy of nutritional therapy with exclusive enteral nutrition (EEN) to induce remission with an excellent safety profile. However, EEN is poorly tolerated, limiting its compliance and clinical application. This has spiked an interest in the development of alternative and better-tolerated nutritional therapy strategies. Several nutritional therapies have now been designed not only to treat the nutritional deficiencies seen in children with active CD but also to correct dysbiosis and reduce intestinal inflammation. In this review, we report the most recent insights regarding nutritional strategies in children with active CD: EEN, partial enteral nutrition (PEN), Crohn’s disease exclusion diet (CDED), and CD treatment-with-eating diet (CD-TREAT). We describe their setup, efficacy, safety, and (dis)advantages as well as some of their potential mechanisms of action and perspectives. A better understanding of different nutritional therapeutic options and their mechanisms will yield better and safer management strategies for children with CD and may address the barriers and limitations of current strategies in children.

Summary： Background：The continuous outbreak of COVID-19 poses a devastating threat to the global public health, and there's no special therapeutic drugs.This paper is to explore the effect of high fiber whey short peptide enteral nutrition on the prognosis of patients with COVID-19, to find ways to prevent patients from progressing to severe illness, and to reduce the harm of epidemic situation to human beings. Methods: The course of fighting against COVID-19 in our hospital for 49 days was reviewed. Three nutritional interventions including five-step nutrition treatment, early intervention of whole protein, and high fiber whey short peptide intervention were conducted consecutively.The effect of high fiber whey short peptide on nutrition, immune function and prognosis of patients with COVID-19 was compared with that of whole protein intervention. Results: High fiber whey short peptide group when compared with whey protein group, can significantly improve prealbumin in patients with covid-19, improve the negative nitrogen balance of patients, and reduce the average period of turning negative by 39.06%. Conclusion: High fiber whey short peptide can significantly improve the recovery speed of patients with covid-19.

Esta revisión se basa en la mejor evidencia actualmente disponible, y en ella se definen las directrices en relación con la evaluación y manejo nutricional en niños críticamente enfermos. Estas directrices incluyen, los criterios para la detección de la malnutrición, tanto por déficit o exceso, así como, se identifican las recomendaciones y consensos de expertos sobre la estimación energética y de macronutrientes, los cuales pueden ser determinantes en prevenir tanto la sobrealimentación como subalimentación. También se señalan las recomendaciones internacionales sobre el momento de inicio del soporte nutricional para que esta sea oportuna, efectiva y segura, eligiendo la vía de alimentación según condición clínica y tipo de patología en función de disminuir el riesgo de morbimortalidad de estos pacientes. A modo general, se sugiere intensificar la investigación científica, con el fin de disponer de mejor evidencia para diseñar protocolos clínicos internacionales y locales para el manejo nutricional del paciente pediátrico críticamente enfermo.
