Classification of temporomandibular joint erosion, arthritis, and inflammation in patients with juvenile idiopathic arthritis

Abstract: This is the first classification system to link CBCT and MRI with the use of contrast medium. Contrast-enhanced MRI is an internationally recognized technique that permits acute inflammation to be unequivocally diagnosed. Although structural erosion of the TMJs in our arthritis group was generally severe and significant, we were surprised to observe some cases that were clinically asymptomatic.

“…Oxidative stress was increased in JIA patients (Breda et al 2013;Lipinska et al 2015). On the other hand, JIA is the most common disease in pediatric rheumatology and characterized by chronically joint destruction and inflammation (Koos et al 2013;Abramowicz et al 2013). Fortunately, anti-oxidant and anti-inflammation properties of blueberry have been widely reported with fewer side effects (Kraujalyte et al 2015;Chiabrando and Giacalone 2015;Diaconeasa et al 2015;Kang et al 2015).…”

“…For the purposes of the study, inflammation of the TMJ (active TMJ arthritis) was defined as excessive synovial Gd enhancement and synovial hypertrophy, whereby a line less than 0.5 mm thick was considered normal, as in other studies published in this field 15,16,17,18,19 . The extent of the inflammation or TMJ damage (flattening or destruction of the condyles) was not taken into account in our study.…”

“…16,69,89 However, both the terminology and the definitions of the various joint pathologies as expressed by MRI have been inconsistently used and reported, thus giving the need for expert groups to develop generally accepted definitions. Longitudinal, prospective studies must be performed in the future to evaluate changes in the MRI-assessed pathologies of the TMJ in patients with JIA over time.…”
