Classification and pathology of primary progressive aphasia

Abstract: The 2011 PPA recommendations classify a large proportion of patients who meet basic PPA criteria. However, some patients had aphasic syndromes that could not be classified, suggesting that the 2011 recommendations do not cover the full range of PPA variants. Classification of semantic variant PPA provides a good prediction of underlying pathology. Classification of logopenic variant does not successfully differentiate PPA due to AD from PPA due to other pathologies.

“…4 Behavioural variant (bv) of FTD was diagnosed by revised diagnostic criteria reported by the International bvFTD Criteria Consortium 5 and language variant of FTD was diagnosed by latest published criteria. 6 Patients with VaD were diagnosed according to the criteria of the NINDS-AIREN (National Institute of Neurological Disorders and Stroke/Association Internationale pour la Recherche et l'Enseignement en Neurosciences). 18 In this study, we reviewed the medical records of eligible subjects and collected data including age, sex, education level, Mini-Mental State Examination score, Clinical Dementia Rating scale score, molecular imaging report including the standardised uptake value ratio (SUVR) of 11 C-PIB PET, and the pre-and post-imaging diagnoses.…”

“…However, some studies have been published concerning the usefulness of the classification and these criteria, with controversial results [5,6,7]. Positron emission tomography imaging with 18 F-fluorodeoxyglucose (FDG-PET) aids in the topographic diagnosis, where hypometabolism is associated with neurodegeneration [8].…”

“…The sv-PPA is strongly associated with microscopic inclusions of transactive response DNAbinding protein of 43 kDa (TDP-43), the nfv-PPA with tau inclusions, and a large proportion of lv-PPA cases with Alzheimer's pathology. [28][29][30] Despite these seemingly clear-cut associations, a number of clinical and pathological series demonstrate that some cases show unexpected pathological associations and not all PPA cases can be classifiable into one of the three variants. This raises issues about the biological validity and applicability of this tripartite approach.…”
