ObjectiveTo assess the correlation between different pain symptoms and different domains of women's health-related quality of life (HRQoL).MethodsSeventy-seven women with deep infiltrating endometriosis were successively enrolled between June 2011 and August 2013 while being prepared to undergo laparoscopy due to pain and/or infertility. We quantified the intensities of dysmenorrhea, deep dyspareunia, chronic pelvic pain, and dyschezia (menstrual and non-menstrual) using a 11-point visual analog scale (VAS: 0–10) and the validated full versions of the Short Form 36 (SF36) and Endometriosis Health Profile (EHP30) questionnaires to assess HRQoL. The pain symptoms were considered simultaneously in a hierarchical agglomerative clustering method (exploratory multivariate approach) and the associations among scores were tested by bivariate correlation.ResultsDysmenorrhea showed the lowest similarity on to the multivariate cluster analysis and no statistically significant correlation with the other pain symptoms: deep dyspareunia (P=0.244), chronic pelvic pain (P=0.108), menstrual dyschezia (P=0.238), and non-menstrual dyschezia (P=0.380). Dysmenorrhea and chronic pelvic pain were the main symptoms correlated with all domains of the SF36 and the EHP30 (core instrument) questionnaires (P<0.05).ConclusionDysmenorrhea and chronic pelvic pain were independent factors associated with HRQoL.

Purpose To evaluate the effect of mesenchymal stem cells (MSCs) on fertility in experimental retrocervical endometriosis. Methods A total of 27 New Zealand rabbits were divided into three groups: endometriosis, in which endometrial implants were created; mesenchymal, in which MSCs were applied in addition to the creation of endometrial implants; and control, the group without endometriosis. Fisher's exact test was performed to compare the dichotomous qualitative variables among the groups. The quantitative variables were compared by the nonparametric Mann-Whitney and Kruskal-Wallis tests. The MannWhitney test was used for post-hoc multiple comparison with Boniferroni correction. Results Regarding the beginning of the fertile period, the three groups had medians of 14 AE 12.7, 40 AE 5, and 33 AE 8.9 days respectively (p ¼ 0.005). With regard to fertility (number of pregnancies), the endometriosis and control groups showed a rate of 77.78%, whereas the mesenchymal group showed a rate of 11.20% (p ¼ 0.015). No differences in Keenan's histological classification were observed among the groups (p ¼ 0.730). With regard to the macroscopic appearance of the lesions, the mesenchymal group showed the most pelvic adhesions. Conclusion The use of MSCs in endometriosis negatively contributed to fertility, suggesting the role of these cells in the development of this disease.

A endometriose é um distúrbio inflamatório estrogênio-dependente, caracterizada pelo crescimento exacerbado de tecido endometrial e células do estroma fora da cavidade uterina. A qualidade de vida é diminuída em mulheres com endometriose, principalmente com o agravamento dos sintomas. Tendo em vista a importância da endometriose para saúde pública, o presente artigo teve por objetivo explanar a fisiopatologia da endometriose, as formas de diagnóstico e tratamento, bem como as implicações da doença na fertilidade feminina e formas de tratamento através da reprodução assistida. Para tanto, foram selecionadas 50 referências de língua portuguesa e inglesa datadas entre os anos de 2014 a 2020, prioritariamente. A etiologia da endometriose é complexa, cercada por diversas teorias, e o diagnóstico precoce é crucial para a minimização dos danos à fertilidade, visto que 30 a 50% das mulheres com endometriose exibem quadro de infertilidade. São observadas diversas complicações em pacientes acometidas pela patologia que levam à infertilidade. A fim de manter a fertilidade feminina, técnicas de reprodução assistida têm sido utilizadas com sucesso. Diante do exposto, conclui-se que para a manutenção da qualidade de vida e fertilidade feminina, o diagnóstico precoce da endometriose é imprescindível. Além disso, as técnicas de reprodução assistida surgem como a linha de tratamento com maior sucesso terapêutico para mulheres acometidas por endometriose que desejam engravidar.
