Civilian gunshot wounds to the head

Abstract: Previous retrospective studies of cranial gunshot wounds have failed to determine whether aggressive field resuscitation, triage to a neurosurgical center, and early surgical intervention can improve the assumed poor outcome of these severely injured patients. Therefore, we studied 100 consecutive patients prospectively to establish a systematic approach to treatment. If the patient retained two or more neurological signs after aggressive field resuscitation/intubation, a computed tomographic scan was performe… Show more

“…On the other hand, all patients with a GCS score > 5 should receive aggressive surgical therapy. [7] More recent studies have recommended aggressive surgical management in patients with GCS scores ≥ 8. [614] Our results differ greatly from those reported by Clark et al ., as 3 out of 4 of our patients had a GSC of 3, received emergent surgical decompression and survived.…”

“…Early and adequate surgical debridement of the necrotic tissue reduces both the incidence of elevated intracranial pressure and the rate of infection [9]. Intracranial infections are a feared complication of penetrating missile injuries to the brain.…”

“…Based on the outcomes of retrospective studies from early military and more recent civilian series, physicians have assumed that patients who have sustained a gunshot to the head resulting in a transventricular injury, an injury that crosses the midline, or a low admitting Glasgow Coma Scale score have a poor prognosis. Violence, commonly involving gunshot wounds, particularly gunshot wounds to the head, is a major and worsening public health problem [9, 12]. …”
