BACKGROUND: Fecal microbiota transplantation (FMT) is an important therapeutic option for recurrent or refractory Clostridioides difficile infection, being a safe and effective method. Initial results suggest that FMT also plays an important role in other conditions whose pathogenesis involves alteration of the intestinal microbiota. However, its systematized use is not widespread, especially in Brazil. In the last decade, multiple reports and several cases emerged using different protocols for FMT, without standardization of methods and with variable response rates. In Brazil, few isolated cases of FMT have been reported without the implantation of a Fecal Microbiota Transplantation Center (FMTC). OBJECTIVE: The main objective of this study is to describe the process of implanting a FMTC with a stool bank, in a Brazilian university hospital for treatment of recurrent and refractory C. difficile infection. METHODS: The center was structured within the criteria required by international organizations such as the Food and Drug Administration, the European Fecal Microbiota Transplant Group and in line with national epidemiological and regulatory aspects. RESULTS: A whole platform involved in structuring a transplant center with stool bank was established. The criteria for donor selection, processing and storage of samples, handling of recipients before and after the procedure, routes of administration, short and long-term follow-up of transplant patients were determined. Donor selection was conducted in three stages: pre-screening, clinical evaluation and laboratory screening. Most of the candidates were excluded in the first (75.4%) and second stage (72.7%). The main clinical exclusion criteria were: recent acute diarrhea, overweight (body mass index ≥25 kg/m2) and chronic gastrointestinal disorders. Four of the 134 candidates were selected after full screening, with a donor detection rate of 3%. CONCLUSION: The implantation of a transplant center, unprecedented in our country, allows the access of patients with recurrent or refractory C. difficile infection to innovative, safe treatment, with a high success rate and little available in Brazil. Proper selection of qualified donors is vital in the process of implementing a FMTC. The rigorous clinical evaluation of donors allowed the rational use of resources. A transplant center enables treatment on demand, on a larger scale, less personalized, with more security and traceability. This protocol provides subsidies for conducting FMT in emerging countries.

Human cytomegalovirus (CMV) is a widespread persistent herpes virus requiring lifelong immune surveillance to maintain latency. Such long-term interactions with the immune system may be associated with deleterious effects including immune exhaustion and senescence. Regarding the COVID-19 pandemic, we asked whether CMV-specific cellular and humoral activity could influence immune responses toward SARS-CoV-2 and/or disease severity. All adults with mild ( n = 15) and severe ( n = 14) COVID-19 were seropositive for anti-CMV IgG, but negative for IgM antibodies. Antibody titers did not correlate with COVID-19 severity. Six patients presented elevated frequencies of CMV-specific CD4 + and CD8 + T cells producing IFNγ, IL-17, and TNFα, designated as CMV high responders (hiT CMV). In comparison to low CMV responders, hiT CMV individuals exhibited higher frequencies of SARS-CoV-2-specific CD4 + IL-17 + and CD8 + IFNγ + , IL-17 + or TNFα + T cells. These results indicate that high frequencies of CMV-specific T cells may be associated with a SARS-CoV-2-reactive profile skewed toward Th17-dominated immunity. O citomegalovírus, também conhecido como HHV-5, é um herpesvírus pertencente à família Herpesviridae, (subfamília β-Herpesvirida). A obtenção ou infecção primária é ocasionada através da introdução do vírus na célula que possui preferência pelos fibroblastos. A transmissão é inter-humana, por meio do contato direto com secreções corporais contaminadas, tais como: saliva, urina, leite materno, sêmen, secreções vaginais e respiratórias, fezes, sangue e lágrima. Durante a gestação dá-se o nome de transmissão vertical, podendo ocorrer nos períodos: pré-natal, perinatal ou pós-natal. Este trabalho, tem por objetivo, analisar as (re)infecções por Citomegalovírus em gestantes. Trata-se de um estudo secundário, classificado como revisão narrativa, compreendendo série histórica entre 2011 a 2021 nas bases de dados da SCIELO, LILACS, PubMed, BDENF, MEDLINE e GOOGLE ACADÊMICO. Após a aplicação dos filtros, foram encontrados 12 arquivos para análise narrativa agrupados nos seguintes pilares: 1) Evidências sobre as infecções por Citomegalovírus e sinais e sintomas prevalentes no decorrer da gravidez; 2) Rastreamento; e 3) Repercussões do vírus em recém-nascidos. Os estudos demonstraram que os testes sorológicos para IgM e IgG específico, além do teste de avidez para IgG, representam os métodos mais confiáveis de diagnóstico da infecção materna. A icterícia e a perda auditiva prevaleceram como consequências do vírus em neonatos de acordo com as citações. A educação em saúde é o método mais eficaz contra a infecção por CMV.
