Pediatric surgery receives great demand for referrals from primary care services in order to evaluate the need for surgical intervention. However access to this specialized evaluation and in intervention does not always occur at the appropriate time. This study aims to characterize the profile of pediatric patients electively operated in the western Paraná state region, between 2018 and 2020, and identify those who were lately referred to surgical evaluation. This is a descriptive, cross-sectional and retrospective study through the review of electronic medical records. The variables evaluated were sociodemographic data, information on underlying diseases, referral data, specialist assessment and surgical procedure. During this period, 410 patients underwent an elective surgical procedure, of which 289 were included in the research. The sample was predominantly male (72.3%) with a mean age of 57.9 months at the surgeons assessment and 59 months at the date of surgery. Most of the patients came from primary care (75%) and the most common pathology was inguinal hernia (39.1%). The mean time interval between referral through primary care and surgery was 4.98 months, and between the surgeons assessment and surgery was 1.21 months. Of the total sample, 77 (26.6%) patients were identified as being referred late for the surgical procedure. Knowing the profile of patients and the problems experienced in this region in relation to the care provided in pediatric surgery provides subsidies to propose improvement strategies not only for the health system in this location, but for several inner regions of Brazil in a similar situation.

BackgroundThis study assesses the parents' Willingness To Pay (WTP) for One Stop Anesthesia (OSA). OSA is part of a free screening procedure that determines the timing of the anesthesiological assessment. In OSA-positive patients, the preoperative assessment is carried out on the same day as the surgery. The OSA allows patients who have to undergo surgery in a pediatric day surgery to avoid accessing the pre-admission clinic.MethodThis is a descriptive cohort study. A sample of 106 parents were interviewed directly by means of a questionnaire. The questionnaire builds a hypothetical scenario where the interviewee has a chance to buy the OSA health service with the WTP. The WTP values are distributed in classes and are contingent to the market built in the questionnaire. The Chi Square and Cramer's V tests evaluate the WTP dependence on the parents' place of origin and occupation.ResultsThe approximate average of the WTP classes is €87.21 per family. The Chi Square test relative to the WTP classes and the places of origin is statistically significant (p < 0.05). The Cramer's V test is 0.347 and points to a positive association between the two demographics. The Cramer's V test of the WTP classes and the types of job is 0.339 and indicates a positive association.ConclusionNearly 90% of pediatric patients who were screened for timing the preoperative assessment are true positives to OSA. This allows doing away with the pre-hospitalization, with definite advantages for the families. This screening is a health service that families would be hypothetically willing to pay.

Este estudo objetivou analisar a demanda da cirurgia pediátrica e fatores associados à inadequação da idade cirúrgica. Trata-se de estudo transversal, com aplicação de questionários, em cidade polo do norte de Minas Gerais, Brasil. A associação entre a inadequação da idade cirúrgica foi verificada para fatores demográficos, sociais e assistenciais por meio de análises bivariadas, seguidas de regressão de Poisson, com variância robusta, assumindo-se um nível de significância de 5%. Participaram 532 crianças, 79.3% do sexo masculino. O intervalo médio entre o encaminhamento inicial e a cirurgia foi de 9.2 meses. A frequência de idade inadequada para intervenção cirúrgica foi de 44.4%. As variáveis associadas com a inadequação foram sexo masculino (RP = 1.09; IC95% = 1.01-1.18), baixa escolaridade materna (RP = 1.13; IC95% = 1.06-1.20) e idade de percepção da afecção superior a 1 ano de idade (RP = 1.09; IC95% = 1.02-1.15). Há uma alta ocorrência na inadequação da idade para os procedimentos cirúrgico e entre os fatores associados destacam-se as condições sociais das famílias. Estratégias devem ser elaboradas para reduzir o tempo de espera para crianças com necessidade de intervenção cirúrgica.
