Circulating androgens in male patients suffering from systemic scleroderma

Abstract: The excretion of collagen metabolites and circulating androgens was measured in ten males suffering from progressive systemic sclerosis. Significantly higher levels of sex hormone-binding globulin, total testosterone and dihydrotestosterone (DHT) (P less than 0.01), and total oestradiol (P less than 0.05), were found in patients when compared with age-matched controls. Urinary excretion of hydroxyproline in patients was found to correlate significantly with total testosterone (P = 0.035), with DHT (P = 0.005) … Show more

“…It is unclear whether free or total serum testosterone was measured in this study. However, a slightly larger study demonstrated significantly higher total serum testosterone and E2 levels in 10 male dcSSc patients compared to age-matched controls [29]. Thus, increased testosterone levels in dcSSc males, as in healthy males, likely drive the elevated serum E2 levels and may explain the difference we observed between dcSSc male and female patients.…”

“…In male SSc patients, we observed significantly lower levels of both total testosterone and bioactive testosterone, as compared to age‐matched controls. Two previous studies did not detect any alteration in sex hormone levels in male SSc patients, but those studies were limited by their sample size, while another study showed that the levels of testosterone and SHBG were higher in male SSc patients compared to controls. This may be attributable to differences in sample size and inclusion criteria, since one of the studies included patients with rheumatoid arthritis among the controls, and excluded SSc patients based on the presence of additional comorbidities, including 1 patient with Klinefelter's syndrome.…”

“…Existem apenas alguns relatos de casos de esclerodermia em pacientes com síndrome de Klinefelter 21,22 , e foi descrito que homens com esclerodermia sistêmica apresentam ní-veis mais elevados de estradiol circulante 23 . Em nossa casuística, houve correlação negativa entre o tempo de doença e a densidade óssea apenas no colo do fêmur, não ocorrendo o mesmo nas outras regiões analisadas.…”
