Deep venous thrombosis (DVT) is a common complication in patients with lower extremity fractures, causing delays in recovery short-term and possible impacts on quality of life long-term. Early prediction and prevention of thrombosis can effectively reduce patient pain while improving outcomes. Although research on the risk factors for thrombosis is prevalent, there is a stark lack of clinical predictive models for DVT occurrence specifically in patients with lower limb fractures. In this study, we aim to propose a new thrombus prediction model for lower extremity fracture patients. Data from 3300 patients with lower limb fractures were collected from Wuhan Union Hospital and Hebei Third Hospital, China. Patients who met our inclusion criteria were divided into a thrombosis and a nonthrombosis group. A multivariate logistic regression analysis was carried out to identify predictors with obvious effects, and the corresponding formulas were used to establish the model. Model performance was evaluated using a discrimination and correction curve. 2662 patients were included in the regression analysis, with 1666 in the thrombosis group and 996 in the nonthrombosis group. Predictive factors included age, Body Mass Index (BMI), fracture-fixation types, energy of impact at the time of injury, blood transfusion during hospitalization, and use of anticoagulant drugs. The discriminative ability of the model was verified using the C-statistic (0.676). For the convenience of clinical use, a score table and nomogram were compiled. Data from two centers were used to establish a novel thrombus prediction model specific for patients with lower limb fractures, with verified predictive ability.

Purpose: was demonstrated to be aberrantly expressed and affected cell activities in some types of diseases, including cancer. However, the role of miR-4284 in non-small cell lung cancer (NSCLC) is largely unknown. The aim of this study was to investigate the expression and biological role of miR-4284 in NSCLC. Patients and Methods: The qRT-PCR assay was applied to detect the expression of miR-4284 in NSCLC tissues and cell lines. Kaplan-Meier curve method and multiple Cox regression analyses were used to explore the prognostic factors for postoperative NSCLC patients. The CCK-8 assay was carried out to measure the proliferative abilities of A549 and H1299 cells. Transwell migration and invasion assays were used to determine the cell migratory and invasive capabilities of NSCLC cells. Results: miR-4284 expression was upregulated in NSCLC tissues and cell lines. High expression of miR-4284 was correlated with poor differentiation, positive lymph node metastasis, and advanced TNM stages. In addition, postoperative patients with higher expression of miR-4284 exhibited a shorter overall survival time than those with lower expression of miR-4284. Moreover, the upregulation of miR-4284 accelerated cell proliferative, migratory, and invasive abilities of A549 and H1299 cells, while the downregulation of miR-4284 inhibited these cellular capabilities. Conclusion: miR-4284 was noticeably upregulated in NSCLC and associated with a poor prognosis of postoperative NSCLC patients. miR-4284 promoted the proliferation, migration, and invasion of A549 and H1299 cells. This study indicated that miR-4284 might serve as a prognostic biomarker and a potential therapeutic target for postoperative NSCLC patients.

Safety of Dabigatran as an Anticoagulant: A Systematic Review and Meta-Analysis

Background: Dabigatran is a univalent low-molecular-weight direct thrombin inhibitor which was developed as an alternative to vitamin K antagonists (VKAs). However, the safety of dabigatran remains controversial so far. In this study, we aimed to compare the risk of bleeding, fatal adverse events, and the all-cause mortality of dabigatran with those of the control group by a systematic review and meta-analysis of randomized controlled trials.Methods: We systematically searched PubMed, Web of Science, Cochrane Library, Medline, Embase, Wanfang database, Clinical trial, China National Knowledge Infrastructure Chinese Scientific Journal database (VIP), and Chinese Biological Medicine database (CBM), for clinical trials on conventional treatments compared with dabigatran, published between January 2014 and July 2020. The reported outcomes, including the endpoints of primary safety, were systematically investigated.Results: Seven RCTs (n = 10,743) were included in the present systematic review. Compared to the control groups, dabigatran was not associated with an increased risk of major bleeding (relative risk [RR] 0.86, 95% confidence interval [CI]: 0.61 to 1.21, p = 0.06), intracranial hemorrhage (RR 0.89, 95% CI: 0.58 to 1.36, p = 0.41), fatal adverse reactions (RR 0.87, 95% CI: 0.65 to 1.17, p = 0.66), all-cause mortality (RR 0.88, 95% CI: 0.70 to 1.11, p = 0.45, I2 = 0%), and significantly reduced risk of clinically relevant non-major bleeding (RR 0.96, 95% CI: 0.65 to 1.42, p = 0.0007). However, dabigatran is associated with an increased risk of gastrointestinal (GI) bleeding (RR 1.78, 95% CI: 1.02 to 3.13, p = 0.05).Conclusion: Dabigatran has a favorable safety profile in terms of major bleeding, intracranial hemorrhage, and life-threatening events, among other safety outcomes. The present study suggested that dabigatran may be a suitable alternative to VKAs as an oral anticoagulant. However, more data are necessary to clarify the incidence of other adverse events and serious adverse reactions.
