Purpose: To investigate the effect and mechanism of action of dexmedetomidine (Dex) on podocyte injury. Methods: Cells were incubated with high glucose (50 mM) to induce a podocyte injury model in vitro. Cell viability, apoptosis, the expression of related protein related in podocyte injury and albumin permeability were evaluated by 3-(4, 5-dimethylthiazol-2-yl)-2, 5-diphenyl tetrazolium bromide (MTT), flow cytometry, western blot and Transwell assays. Results: Dex administration enhanced HG-induced cell viability and the relative protein expression of Bcl-2, but reduced the HG-induced relative protein level of Bax and apoptosisrate in podocytes (p < 0.05). Besides, Dex incubation compensated HG-induced relative protein expressions of nephrin and podocin in podocytes but did the reverse with regard to relative protein expression of desmin and albumin permeability (p < 0.05). Moreover, Dex treatment resulted in a decrease in ectodysplasin A2 receptor (EDA2R) expression in HG-induced podocytes. The level of EDA2R was upregulated by the transfection of overexpression plasmid containing the EDA2R sequences. Overexpression of EDA2R reversed Dex-induced increase in cell viability, apoptosis, expression of nephrin, podocin and desmin, as well as albumin permeability in HG-stimulated podocytes (p < 0.05). Conclusion: Dex ameliorates HG-induced podocyte injury via inhibition of EDA2R, indicating that Dex is a potential alternative drug for the treatment of podocyte injury.

Background Cerebral ischemia-reperfusion injury (CIRI) is an important factor affecting the prognosis of patients with ischemic stroke. This study aimed to observe whether isoflurane posttreatment enhances autophagy after focal CIRI in rats by activating the AMPK/ULK1 signaling pathway, thereby inhibiting NLRP3 inflammation. The body releases inflammatory factors to reduce inflammation, thereby reducing neurological damage and improving cognitive and memory functions. Methods Eighty male SD rats were randomly divided into 5 groups: sham operation group (Sham), model group (MCAO), isoflurane posttreatment + model group (M + ISO), autophagy inhibitor Baf-A1 + isoflurane posttreatment + model group (M + I + B), AMPK inhibitor Compound C + isoflurane posttreatment + model group (M + I + C). Results Compared with the sham group, the MCAO group exhibited decreased neurobehavioral scores and cognitive memory function (P < 0.05). Compared with the MCAO group, the neurobehavioral score of rats in the M + ISO group was significantly reduced, the expression of AMPK, ULK1, Beclin1, and LC3B protein was significantly increased, and cognitive and memory function was significantly improved (P < 0.05). Compared with the M + ISO group, the neurobehavioral scores and the protein expression of NLRP3, IL-1β and IL-18 in the M + I + B and M + I + C groups increased significantly (P < 0.05). Conclusions This research indicated that isoflurane posttreatment may enhance autophagy by activating the AMPK/ULK1 signaling pathway and further inhibit the release of inflammatory factors from NLRP3 inflammasomes, thereby improving neurological function and cognitive impairment after focal CIRI in rats and exerting a protective effect on the brain.

PurposeTo compare the effects of remimazolam and dexmedetomidine on early postoperative cognitive dysfunction (POCD) in aged gastric cancer patients.MethodsFrom June to December 2022, 104 elderly patients (aged 65–80 years) received laparoscopic radical resection of gastric cancer at the First Affiliated Hospital of Nanchang University. Using the random number table approach, the patients were separated into three groups: remimazolam (Group R), dexmedetomidine (Group D), and saline (Group C). The primary outcome was the incidence of POCD, and secondary outcomes included TNF-α and S-100β protein concentrations, hemodynamics, VAS scores, anesthesia recovery indicators, and the occurrence of adverse events within 48 h postoperatively.ResultsAt 3 and 7 days after surgery, there were no statistically significant differences in the incidence of POCD, the MMSE and MoCA scores between groups R and D (p > 0.05). However, compared to the saline group, both groups had higher MMSE and MoCA scores and decreased incidences of POCD. These differences were statistically significant (p < 0.05). Between group R and group D, there were no statistically significant changes (p > 0.05) in the levels of TNF-α and S-100β protein at the three time points (at the end of the surgery, 1 day later, and 3 days later). Even though neither group’s concentration of the two factors was as high as that of the saline group, the differences were statistically significant (p < 0.05). At all three time points—following induction (T2), 30 min into the operation (T3), and at the conclusion of the surgery (T4)—the heart rate and blood pressure in group R were greater than those in groups D and C. Statistics showed that the differences were significant (p < 0.05). The incidence of intraoperative hypotension was highest in group D and lowest in group R (p < 0.05). The dose of propofol and remifentanil, group C > group R > group D. Extubation and PACU residence times did not differ statistically significantly (p > 0.05) between the three groups. There was no significant difference in VAS scores between groups R and D after 24 h postoperatively (p > 0.05), although both had lower scores than group C, and the difference was statistically significant (p < 0.05). The VAS scores between the three groups at 72 h (T6) and 7 days (T7) were not statistically significant (p > 0.05). Adverse reactions such as respiratory depression, hypotension, bradycardia, agitation, drowsiness, and nausea and vomiting had the lowest incidence in group R and the highest incidence in group C (p < 0.05).ConclusionRemimazolam is similarly beneficial as dexmedetomidine in lowering the incidence of early POCD in aged patients after radical gastric cancer resection, probably due to reduced inflammatory response.
