Ciguatera poisoning

Abstract: Ciguatera is a form of poisoning that occurs after eating tropical and subtropical ciguatoxic fish. The ciguatoxins are a family of heat stable, lipid soluble cyclic polyether compounds that bind to and open voltage-sensitive Na(+) channels at resting membrane potential, resulting in neural hyperexcitability, as well as swelling of the nodes of Ranvier. The authors describe a 45-year-old man who developed acute gastrointestinal symptoms in Antigua soon after eating red snapper and grouper, potentially "ciguato… Show more

“…The proposed mechanism reported in previous experimental studies was to bind to and open voltage-gated sensitive Na + channels at resting membrane potential, resulting in neural hyperexcitability. Followed by continuous Na + influx, cells become swollen because water enters the cell throughout the effect to maintain the osmotic equilibrium between intra- and extracellular space 1 3. The mechanism was proven partially by the finding of the sural nerve oedema on biopsy 4…”

“…Fishery products supply millions of tons of fish worldwide to feed low-income populations of costal and insular areas, and provide an alternative protein source in Europe and America. This demand exerts rising pressure on fishing grounds, and in combination with globalization of trade, is leading to increased import and greater consumption of fish sourced from areas where ciguatera is endemic (Achaibar et al, 2007;Garcia and Rosenberg, 2010). Thus, the disease is no longer restricted to the South and Equatorial Pacific, the Caribbean Sea and the Indian Ocean (Bagnis et al, 1979;Lange, 1994), where several hundreds of fish species are known to harbor the toxins responsible for ciguatera.…”

“…Most of the cases are reported from countries within and surrounding the Pacific and Indian Oceans and the Caribbean. 4 Although most ciguatera cases are seen in endemic tropical areas, there is an increasing potential for cases to be encountered in temperate regions, as recently described. 5 In this study, we aim to describe two clusters of collective common-source CFP in Paris due to consumption of two fish imported from Guadeloupe (French West Indies).…”

“…3 CFP is a significant public health issue in endemic areas that extend between 35 ∘ N and 35 ∘ S, approximately. 4 CFP is probably mostly undiagnosed even if its worldwide incidence is estimated to be around 50,000 cases annually. CFP is the most common poisoning linked to the consumption of finfish.…”
