Chylous Bronchial Casts After Fontan Operation

Abstract: A 4-year old boy with D-transposition of the great arteries, pulmonary atresia with intact ventricular septum, and a hypoplastic left ventricle with mitral atresia underwent a Fontan operation. He had previously been treated with neonatal balloon atrial septostomy followed by a left modified Blalock-Taussig shunt and, at the age of 2 years, a Hemi-Fontan operation was performed. The Fontan operation was completed with an extracardiac conduit (Goretex) from the inferior vena cava to the superior vena cava. High… Show more

“…Endobronchial chyle leakage has also been reported to cause plastic bronchitis in a postoperative Fontan patient after pleurodesis for persistent chylothoraces. 21 Although our patient did not have elevated systemic venous pressures or evidence of Fontan pathway obstruction at recent cardiac catheterization, the majority of Fontan patients who are doing poorly as a result of pump failure or refractory atrial arrhythmias have significant hemodynamic derangements. 23 Of these 12 palliated sin- gle-ventricle patients, 6 had died and a seventh was awaiting heart transplantation at the time that these cases were reported.…”

“…Lymphatic involvement has been considered to be part of the pathological process in PB because of the dilation of the lymphatic vessels found at lung biopsies and the abnormal lung tracer intake shown by lymphoscintigraphy. [5][6][7] Poor understanding of the pathogenesis of the disease has resulted in variability in therapies across centers. Interventions aimed at lowering venous pressure have been reported to reduce cast formation.…”

“…In children with CHD after Fontan procedure, abnormal lymphatic system function, coupled with increased CVP and endobronchial lymphatic leakage, are probable mechanisms involved in causing PB [2]. The casts may reflect mucus hypersecretion as a response of the respiratory epithelium to increased pulmonary venous pressure.…”
