Chronological age or biological age: What drives the choice of adjuvant treatment in elderly breast cancer patients?

Abstract: Highlights The choice of adjuvant therapy in elderly patients should consider several factors. There is a lack of universal and unique guidelines to define elderly patients. Biological age is the result of genetics, lifestyle, nutrition and comorbidities. AT choice in elderly patients must consider chronological and biological age. The geriatric assessment and the research results should be evaluated in AT choice.

“…However, this is consistent with our data from OPSCC, suggesting that overall performance status is more important than the physical age of the patient. Also, this is consistent with the suggestion that not only chronological but also biological age and functional status (and probably other factors) should be considered when selecting the best cancer treatment [ 45 , 46 , 47 ].…”

“…These investigations might point to a possible connection between IR and age; however, all of them were done with a focus on chronological age (CA). CA refers to the length of time that a person experiences from birth to the present moment while BA reflects the physiological and functional performance that corresponds to a specific CA, it takes into account the factors such as time elapse, genetics, nutrition, and so on [ 17 ]. Some scholars have recently discovered that BA can better represent a person's physiological function and aging level more than CA, and BA is more consistent with an individual's "true life expectancy" [ 18 ].…”

“…To counter age-biases in research, the National Institute of Health now expects inclusion across the lifespan and explicitly require justification for age exclusions in its funded research portfolio ( 29 ). But nuance is also called for here, as chronological age categories are often distinct from biological/ functional statuses and have different treatment implications ( 30 ).…”
