Chronic ventricular assist device support

Abstract: The first clinical results of the newest generation of VADs are very promising compared with old-generation devices. Furthermore, less invasive surgery is becoming a standard for the implantation, exchange, or explantation of left VADs. The joint venture of improved technology and innovative surgical techniques will push this field forward to even better outcomes and reduced complication rates.

“…Hence a therapy for improvement of cardiac output could enhance renal In endstage chronic heart failure, the gold standard for therapy is still cardiac transplantation however the implantation of an LVAD is a promising alternative. Due to consistently improved technology, e.g., pump size miniaturization and less-LISs, the implantation of LVADs is one of the fastest-developing fields in cardiac surgery (10,17), thus the number of LVAD implantations worldwide has surpassed the number of cardiac transplantations (18). It is known that the major advantages of a less invasive surgical approach are reduction of trauma, reduced blood loss and less infections as well as decreased duration of intensive care unit and in-hospital stay (14,19), but little is known on the impact of LIS on renal function.…”

“…Regional anesthesia is an important component of multimodal analgesia after thoracotomy reducing stress response, pain scores, and opioid consumption 6 . Despite that, the ideal multimodal analgesia regimen should combine regional analgesia and systemic analgesics.…”
