Chronic Urticaria and Celiac Disease: A Case Report

Abstract: We describe a case of a 9-year-old girl who presented chronic urticaria associated with celiac disease. The prevalence of the manifestation of chronic urticaria in celiac disease is unknown but increase in atopic immunologic disorders has been reported in the setting of gluten enteropathy. Relationship between the clinical manifestations is not clear. The present case of subclinical celiac disease diagnosis in an otherwise asymptomatic child with chronic urticaria further reinforces the evidence that different… Show more

“…Other reports associating chronic urticaria with coeliac disease in children have been found. The vast majority of these children had urticaria remission after a gluten-free diet (30)(31)(32). This finding is worthy of further investigation, since it might suggest screening for coeliac disease in children with chronic urticaria, especially in refractory cases.…”

“…Theoretically, this mechanism might cause urticarial lesions, so by restoring the integrity of the mucosa, a GFD might resolve them [ 101 ]. Furthermore, even though no meta-analysis is available, in some cases of CU, the adoption of a GFD has proven effective in controlling skin lesions in CD patients, further confirming that CU may be a cutaneous manifestation of CD and not a mere chance association [ 98 , 102 ]. Recently, Kolkhir et al stated that CU is strongly linked with various autoimmune diseases, such as Hashimoto’s thyroiditis, pernicious anemia, vitiligo, diabetes mellitus type 1, Grave’s disease, and rheumatoid arthritis, and that it is essential to test children with chronic spontaneous urticaria (CSU) for other autoimmune diseases, including CD [ 103 ].…”

“…Celiac disease has been recently recognized as a systemic autoimmune comorbidity in a subset of CSU patients 23 . Anecdotal case reports have reported improvement of CSU after gluten‐free diet (GFD) in patients having concomitant celiac disease 24,25 …”

“…Dietary elimination of fish or fish products and mammalian meat or meat products has Grade B recommendation each, while dietary elimination of gluten has Grade C recommendation (Table 3 ) 16,17,19–22,24–41 . However, elimination diets may be tried in a subset of refractory CSU patients, for example, those with a history of anaphylaxis, resident of tick endemic areas for AGS, or confirmed coexistence of food allergy by a double‐blind placebo‐controlled food challenge, when benefits outweigh risks.…”
