Chronic transmural atrial ablation by using bipolar radiofrequency energy on the beating heart

Abstract: Bipolar radiofrequency energy can produce permanent transmural linear lesions on the beating heart. Online measurement of tissue conductance reliably predicted lesion transmurality. This new technology may enable surgeons to perform a curative minimally invasive operation for atrial fibrillation on the beating heart.

“…Thus, the surgical portion of the procedure was converted to use a bipolar RF clamp (AtriCure Inc., West Chester, OH), which had been shown to be more effective. 1376 This approach provided improved results, and in most cases, gaps in surgical lesions could be completed by endocardial catheter ablation during the same procedure. 608 A sequential hybrid approach was subsequently developed.…”

“…In our centre, bipolar radiofrequency energy has been favoured over other sources, because it can create rapid trans-mural lesions in a reliable and consistent manner (Prasad et al, 2002). These devices use online measurement of conductance as a surrogate for trans-murality.…”

“…Beyond the ''cut and sew'' surgical technique, the conventional use of limited linear and spot ablations in these complex cases, frequently involving suboptimal devices and energy delivery methods, are well-recognized for their deficiencies. 6,7 The epicardial ablation technique described in this study, in combination with the endocardial component, is designed to circumvent many of these limitations and to target in particular an anatomical area recognized as being a dominant and concentrated site of arrhythmia substrate.…”
