Chronic Q fever endocarditis.

Abstract: SUMMARY Eight patients with chronic Q fever endocarditis were treated with tetracycline for up to 40 months. In addition, five of these patients received co-trimoxazole. Six patients had prosthetic valves. Two patients who had Q fever endocarditis on their native valves required valve replacement because of haemodynamic difficulties: in only one did the Q fever endocarditis contribute to the haemodynamic difficulty. One patient died.It is suggested that medical treatment is continued until clinically and haema… Show more

“…In contrast, the Priscilla isolate, implicated as a cause of chronic Q fever in goats (21)-and possibly humans-exhibited significant resistance to all antibiotics that were effective against the Nine Mile isolate. This observation is in agreement with the fact that chronic Q fever in humans, including Q-fever endocarditis, is not successfully managed with either antibiotic treatment or a combination of antibiotic treatment and surgical procedures (7,18,22,24,26,27).…”

“…In fact, doxycycline and rifampin apparently delayed the effect of ciprofioxacin during the first 5 days of treatment (data not shown). DISCUSSION C. burnetii, the etiologic agent of Q fever, was until recently thought to be monogenotypic: acute and chronic disease syndromes were presumed to be the result of patient predisposition (22,25,27). Recent evidence shows distinct plasmid and lipopolysaccharide differences between isolates of C. burnetii derived from animals and humans (14,21).…”

“…Results obtained with combinations including co-trimoxazole are still controversial. Reports (6,7,9,29,34) show that nine patients required valve replacement, since viable C. burnetii were recovered from six patients after therapies of various durations. In our study, one patient was treated with cotrimoxazole combined with doxycycline and died 30 months after initiation of therapy.…”

“…In acute infection, tetracycline shortens the duration of fever by about 50% when the drug is administered within the first 3 days of illness (67), and it remains the drug usually recommended in this setting (4). Cases of endocarditis have been treated with tetracyclines (26,79) or tetracyclines combined with sulfamethoxazoletrimethoprim (27,31,76). A more recent paper described treatment with doxycycline alone and doxycycline combined with rifampin, a quinolone, or sulfamethoxazole-trimethoprim.…”
