“…The presence of local complications is distinct and a defining feature in patients with inflammatory mass of the pancreas, which was interestingly similar to a series reported 30 years back (46% bile duct obstruction, 30% duodenal obstruction, and 15% vascular involvement). 2 Furthermore, almost a quarter of patients have 29 Thus, both the need and choice of surgery is significantly influenced by the initial diagnostic workup and must hence be pursued in all patients depending on clinical presentation.…”

“…Chronic pancreatitis is a pathologic fibro-inflammatory syndrome of the pancreas in individuals with genetic, environmental, and/or other risk factors who develop persistent pathologic responses to parenchymal injury or stress 1 . A small subgroup of CP patients (5%–10%) present as mass lesions, most commonly in the head region of the pancreas, 2 which may either be inflammatory (inflammatory head mass of pancreas [IMP]) or neoplastic, most frequently pancreatic ductal adenocarcinoma (PDAC). This differentiation is particularly important because the patients with CP are also at approximately 15 times higher risk of pancreatic cancer 3 .…”

“…A sub group of patients with CP suffer from an inflammatory mass in the head of the pancreas [23,24] . These mass lesions present great diagnostic and therapeutic dilemas and malignancy needs to be ruled out with certainty.…”

“…Approximately 30% of patients with chronic pancre atitis develop an enlargement of the pancreatic head which can lead to complications such as common bile duct obstruction, obstruction of the main pancreatic duct, vascular obstruction and duodenal compression, all of which represent an indication for surgical treatment [53,54], In the past, the classical Whipple procedure and sub sequently its modification -the pylorus-preserving Whip ple -have served as the standard operations for treating these patients, even though the long-term success of these procedures in patients with chronic pancreatitis has not been convincing [4][5][6], Recently, the appropriateness of these surgical techniques in the treatment of chronic pan creatitis has been questioned. Thus the objective of our randomized, controlled trial comparing the pylorus-pre serving Whipple operation with the duodenum-preserv ing pancreatic head resection was to determine whether there is a demonstrable advantage in the preservation of the duodenum for patients with chronic pancreatitis, especially in light of the fact that the duodenum plays a central role in the postprandial regulation of the digestive process [50,55],…”
