Introduction: Avatars are becoming more common in virtual reality, used as a guide, teacher, companion, or mentor through immersive experiences. Special attention needs to be paid to their design to ensure credibility and working alliance, to allow for the optimal delivery of behavior change content.Methods: We present a new embodied Semi-Autonomous Mentoring Intelligence (SAMI) avatar used in an immersive virtual reality intervention for the self-management of chronic pain. We discuss the research findings that were taken into consideration and guided the design and development of SAMI, such methods to promote working alliance with non-human agents, optimal characteristics of non-human agents, and features of effective “automation”.Conclusion: We provide a table of considerations and recommendations for researchers involved in designing future virtual reality characters. We provide suggestions on how future research could advance SAMI further for use in pain management and related interventions.

Managing pain is essential for social, psychological, physical, and economic reasons. It is also a human right with a growing incidence of untreated and under-treated pain globally. Barriers to diagnosing, assessing, treating, and managing pain are complicated, subjective, and driven by patient, healthcare provider, payer, policy, and regulatory challenges. In addition, conventional treatment methods pose their own challenges including the subjectivity of assessment, lack of therapeutic innovation over the last decade, opioid use disorder and financial access to treatment. Digital health innovations hold much promise in providing complementary solutions to traditional medical interventions and may reduce cost and speed up recovery or adaptation. There is a growing evidence base for the use of digital health in pain assessment, diagnosis, and management. The challenge is not only to develop new technologies and solutions, but to do this within a framework that supports health equity, scalability, socio-cultural consideration, and evidence-based science. The extensive limits to physical personal interaction during the Covid-19 pandemic 2020/21 has proven the possible role of digital health in the field of pain medicine. This paper provides an overview of the use of digital health in pain management and argues for the use of a systemic framework in evaluating the efficacy of digital health solutions.

A dor crónica refere-se a uma dor persistente ou recorrente que dura pelo menos há três meses. A sua prevalência ultrapassa os 30% na população adulta portuguesa. As repercussões da dor crónica afetam a saúde física e mental do indivíduo e estão associadas a grande sofrimento e elevados custos socioeconómicos. A abordagem terapêutica recomendada inclui medidas farmacológicas e não-farmacológicas. Este trabalho tem como objetivo principal a revisão narrativa da literatura em relação à evidência da utilização das terapias cognitivo-comportamentais na dor crónica, inclui uma breve revisão histórica da abordagem psicológica no controlo da dor, a identificação e revisão de abordagens cognitivo-comportamentais apontadas pela evidência como benéficas na dor crónica e uma revisão das novas abordagens que têm surgido e que se revelam promissoras para o futuro da gestão da dor crónica. Os resultados apontam para uma diminuição da sintomatologia álgica crónica após a exposição a sessões de terapia cognitivo-comportamental. É apontada eficácia das terapias tanto de primeira e segunda geração, como de terceira geração, com particular destaque no mindfulness e na terapia de aceitação e compromisso. Começam a surgir também terapias digitais baseadas nos fundamentos cognitivo-comportamentais que parecem soluções promissoras para o futuro. Em suma, as terapias cognitivo-comportamentais demonstraram ser uma abordagem eficaz com benefício na gestão dos quadros álgicos crónicos.
