Chronic pain after hernia repair: a randomized trial comparing Shouldice, Lichtenstein and TAPP

Abstract: Chronic pain after hernia surgery is significantly more common with the open approach to the groin by Shouldice and Lichtenstein methods. The presence of the prosthetic mesh was not associated with significant postoperative complaints. The TAPP repair represents the most effective approach of the three techniques in the hands of an experienced surgeon.

“…[1][2][3][4][5][6] Laparoscopic repair of inguinal hernia has certain advantages over open repair, such as reduced postoperative pain and morbidity, early recovery, rapid return to work, and better quality of life. [3][4][5][6][7] Several strategies have been reported to ameliorate postoperative pain in laparoscopic surgery. 8 The transversus abdominis plane (TAP) block is a regional anesthetic technique that provides analgesia to the parietal peritoneum as well as to the skin and muscles of the anterior abdominal wall.…”

“…Other stated the frequency of chronic pain after inguinal hernia repair to be as high as 54% [12]. To explain this discrepancy, we found that the observational methods for pain evaluation vary, prospective studies are few and chronic pain is not a primary outcome parameter in most studies [11] and [13].…”

“…21 Another well conducted, large, randomized controlled multicenter trial found 10% incidence of chronic pain after laparoscopy and 14% after open surgery in 1983 patients (P <0.05). 22 Another post-operative problem after hernia repair is numbness or hypothesia at the groin. This may be due to injury to cutenous nerve supply.…”
