Chronic iridocyclitis in juvenile rheumatoid arthritis

Abstract: During a 10-year period, chronic iridocyclitis occurred in 8 of 100 patients with juvenile rheumatoid arthritis. It was an initial manifestation in 1 patient and occurred 1-8 years after onset in 7. Three patients had a recurrence. Children at greatest risk have the least arthritis. For early detection of the frequently insidious and recurrent iridocyclitis, regular slit lamp examinations are recommended to help prevent blindness due to band keratopathy and/or cataract.

“…However, after the fourth year of followup, eye surveillance can be less strict. In this study, and in contrast to earlier published results (30), half of our patients who experienced uveitis also had polyarthritis, tending to indicate that these 2 complications, although not associated (P ϭ 0.2), are not exclusive. Our data thus confirmed those of Cimaz and Fink, showing that uveitis occurrence did not influence joint outcome (31).…”

“…Our results support the opinions expressed by authors of earlier reports, that the activity of eye and joint inflammation in children with JRA and associated uveitis tends to be discordant (1,4,5). In 6 of 7 patients described by Schaller et a1 (4), the onset time of uveitis and arthritis differed; 4 patients had the onset of uveitis when arthritis was inactive.…”

“…In most patients with JRA iridocyclitis, the arthritis begins months or a few years before the iridocyclitis (1)(2)(3)(4)(5). Most patients in this series were not tested for ANA prior to the onset of iridocyclitis.…”

“…Iridocyclitis, inflammation of the iris and ciliary body, affects about 10% of children with juvenile rheumatoid arthritis (JRA, Still's disease) and is a major cause of morbidity in the disease (1)(2)(3)(4). Two types of iridocyclitis occur: acute and chronic.…”
