Chronic intestinal pseudo-obstruction in patients with systemic lupus erythematosus: report of four cases

Abstract: Chronic intestinal pseudo-obstruction (CIPO), a recently recognized manifestation of systemic lupus erythematosus (SLE) with only 23 cases reported in the English literature, may appear as a complication or as the initial presentation of SLE and usually occurs during the setting of an active lupus. The pathogenic mechanism in SLE is unknown. We describe four additional cases with clinical, radiological, and manometric features of CIPO. As SLE-related CIPO usually responds to treatment with high doses of cortic… Show more

“…Chronic intestinal pseudo-obstruction (CIP) is a complication that usually occurs during active SLE and is characterised by ineffective intestinal propulsion without any mechanical obstruction of the gut [2] . It is caused by the involvement of visceral smooth muscle and enteric nervous system and can be further classified as idiopathic or secondary.…”

“…5 No specific autoantibodies have been found; however, a few case reports have noted the high prevalence of anti-SSB antibodies in patients with CIPO secondary to SLE. 4,5,9 Our patient had CIPO with haematological involvement as described above, in addition to positive anti-SSB antibodies.…”

“…Also some reports suggest a female preponderance with a mean age of presentation of 40 6. It usually occurs in patients with active SLE but may also be the initial presentation 7. The pathogenesis is not known and is believed to be caused by deposition of immune complex in smooth muscle, enteric nervous system dysfunction or vasculitis, with or without ureteral and vesical involvement 8…”
