Chronic inflammatory demyelinating polyneuropathy: evaluation of the vestibular system with cervical and ocular vestibular evoked myogenic potentials

Abstract: A specific diagnostic protocol including the caloric test, C-VEMPS, O-VEMPS, could be useful when employed for identifying vestibular damage in CIDP patients.

“…39 Since most inner ear gene therapy studies that have successfully improved hearing in mice required neonatal gene delivery ($P2), [27][28][29] it is possible that inner ear gene therapy would need to be delivered in utero for humans. In addition, while many patients with Usher syndrome have significant vestibular dysfunction, 40 it is unclear whether patients with whirlin mutations (DFNB31 or Usher 2D) have vestibular deficits. 9,10,21,[41][42][43] None of the studies that reported on the phenotypes of these patients included detailed vestibular function tests.…”

“…The electrophysiologic investigation with Vestibular Evoked Myogenic Potential (VEMP) has been recently proposed to identify areas of demyelination in the vestibular pathways of MS patients 50 . Furthermore, cervical-VEMPs can exclude the ocular involvement that may modify the sensitivity of VEMPs by interfering with the vestibular responses through ocular fixation compensation by allowing identification of the specific areas affected from demyelination 51 .…”

“…Magliulo et al found 11 of 20 subjects with chronic inflammatory demyelinating neuropathy to have absent or delayed cVEMPs and oVEMPs. 108 However, in another study involving a heterogeneous group of 21 patients with inflammatory neuropathy, VEMP abnormalities tended to be mild, with only 6 patients showing delayed cVEMP or oVEMP latencies. 109 The investigators speculated the milder effects, compared with hereditary polyneuropathy, might represent a milder disease course and/or a difference in the involved pathological mechanisms.…”
