Chronic Hepatitis D; at a Standstill?

Abstract: Chronic hepatitis D (CHD) is a severe liver disease with worldwide distribution caused by the hepatitis D virus (HDV). Therapy of CHD is at a standstill. It still relies on interferon (IFN), introduced empirically in the 1980s; results are limited. With the peghilated IFNs that are now in use, only 25% of CHD reach a sustained viral response, that is, clear the HDV-RNA 6 months after stopping therapy. However, HDV remains infectious and ready to reactivate at very low titers undetectable by current assays, if … Show more

“…Prevalences vary considerably across geographical regions. While HDV infections are rather rare in Europe due to largely effective HBV vaccination programs and screening of blood products, hepatitis D remains of concern with highest rates in low-income HBV-endemic countries with insufficient HBV vaccination coverage 14 . For example, high HDV seroprevalences were reported in Pakistan where vaccination campaigns are difficult to conduct (35.2%) 15 , Mongolia (67%) 16 , Gabon (15.6% to 70.6%) 17 .…”

“…HDV superinfection also is associated with fulminant hepatitis [5] and is a significant concern in HBV/HDV endemic regions and immigrant populations from such regions [6]. This study is the first detailed longitudinal clinical and virological characterization of HDV superinfection in Canada, with the surprising finding of transmission occurring between spouses despite almost a decade of marriage, with 2 children that had been successfully immunized against HBV.…”
