Chronic graft-versus-host disease following umbilical cord blood transplantation: retrospective survey involving 1072 patients in Japan

Abstract: We have little information on chronic graftversus-host disease (GVHD) after cord blood transplantation (CBT). We investigated its clinical features in 1072 Japanese patients with hematologic malignancies who received a transplant through the Japan Cord Blood Bank Network. The primary end point was to investigate the incidence of any chronic GVHD. Median age of the patients was 33 years (range, 0-79 years). The cumulative incidence of chronic GVHD 2 years after transplantation was 28%. Chronic GVHD was fatal in… Show more

“…The reasons for this finding are unclear, but the lower risk of chronic GVHD following UCB grafts in comparison with other hematopoietic stem cell sources may explain, at least in part, these encouraging outcomes. 25,31,32 Cox regression analysis including platelet recovery as a time-dependent variable confirmed the adverse impact on OS although when using this modeling approach, early death is included in failure of platelet recovery.…”

“…15 This study provided detailed information on the prognostic factors of CBT for adult ALL patients. The number of infused CD34-positive cells was associated with engraftment.…”

“…However, few studies have compared the incidences and risk factors of chronic GVHD and its organ-specific symptoms in adult patients receiving U-CB and other available grafts, including related or unrelated BM/PBSC grafts. 16,17 Therefore, we conducted a retrospective study using national registry data involving 4818 patients who underwent allogeneic transplantation. This study aimed to evaluate the incidence and risk factors of chronic GVHD, and the prevalence of chronic GVHD organ involvement in patients who received transplantation using various types of graft, including U-CB.…”
