Chronic expanding hematoma, ruptured through the skin 53 years after buttock contusion

Abstract: INTRODUCTIONChronic expanding hematoma is a relatively rare complication of soft tissue trauma and often clinically mistaken for a malignant neoplasm.PRESENTATION OF CASEA 71-year-old female presented with a chronic expanding hematoma that ruptured through the buttock skin 53 years after the original contusion. The diagnosis of CEH was made based on the results of the biopsy, physical examination, and CT. The tumor was completely excised, and the defect was covered with a rhomboid flap.DISCUSSIONThere are no r… Show more

“…16 There are many reports of CEH in the literature, virtually all of many years duration. [17][18][19] Histologically, these chronic hematomas exhibit a fibrous-walled cyst with a pseudocapsule of variable thickness, often with scattered clusters of histiocytes and hemosiderin deposition; mild chronic inflammation and foci of granulation tissue may be present. 15,16,18 Although the cases described here all conform to the Reid definition of CEH, we required a minimum of 1-year duration to preclude cases of de novo AS presenting as simple hemorrhagic cysts; such cases are typically diagnosed in several months time, and have no circumferential fibrosis.…”

“…16 These fibrous-walled cystic lesions can be found in many bodily sites including extremities, retroperitoneum, pleural cavity, and paraspinal region. [17][18][19] Although typically preceded by trauma, CEH can also develop in the postsurgery setting. Morphologically, CEH is commonly described as a mass of fresh and aging blood or fibrin thrombus debris surrounded by a dense fibrous capsule of variable thickness.…”
