Chromogenic Factor X Assay for Monitoring Warfarin Anticoagulation in a Child With a Prosthetic Mitral Valve

Abstract: Mechanical mitral valve replacement in infants and young children is associated with substantial morbidity and mortality. Lifelong anticoagulation is required, with all the accompanying challenges of maintaining levels in infants and children whose dietary input continually changes. Even with careful control of all aspects that can perturb the coagulation cascade, these patients have a substantial lifelong risk of thrombotic and hemorrhagic complications that can also affect the durability of the valve. Antico… Show more

“…Psychological disorders may induce stress response resulting in the activation of the hypothalamus-pituitary-adrenal axis, the release of adrenocorticotropic hormones, increasing intestinal permeability, and proinflammatory effects on the gastrointestinal tract mediated through macrophages and mast cells, and releasing C-reactive protein [ 139 , 140 ]. Treatment through vedolizumab (anti-TNFα therapy) has been seen to improve depression scores [ 141 ]. Antidepressants might prove to be beneficial through two different pathways: (1) acting on anti-inflammatory effects mediated by the vagus nerve; and (2) directly acting on nitric oxide pathways and factor-κB responsible for the generation of proinflammatory cytokines [ 142 ].…”

“…Another alternative to INR monitoring is monitoring by specific factor levels, in particular factor X. [13][14][15][16][17][18] But again, that approach is not widely used.…”
