Choriocarcinoma of the sigmoid colon: report of a case

Abstract: We report a rare case of primary choriocarcinoma of the colon. A 58-year-old woman underwent Hartmann's procedure to remove a sigmoid colon tumor, and pathological examination confirmed choriocarcinoma that had originated from the colon. Radical surgery combined with chemotherapy gives the best chance of long-term survival.

“…A possible explanation for this differential therapeutic response may be due to different variants of colorectal choriocarcinoma ranging from mixed tumors with a component of conventional intestinal adenocarcinoma to pure trophoblastic neoplasia (14, 24). Interestingly, a combined treatment regimen targeting both components has previously shown efficacy in a case of mixed colorectal choriocarcinoma leading to long-term disease-free survival of 60 months (25). This suggests that histological tumor composition could be a potential indicator of treatment response.…”

“…Choriocarcinoma of extragenital origin has been reported in the retroperitoneum [1], mediastinum [2], and intracalvarium (especially in the pineal gland [3]). When it occurs in the gastrointestinal tract, the stomach is the most common site [4]. PCC originating in the colon is extremely rare, with only 12 documented cases.…”

“…Several hypotheses have been proposed, including that it develops from retained primordial germ cells that migrated abnormally during embryonic development [16]; metastasis from a latent primary lesion in the genitalia [4]; and the retrodifferentiation of pre-existing colonic carcinoma [8]. Among them, the retrodifferentiation theory from pre-existing colonic adenocarcinoma is well accepted.…”

“…A choriocarcinomatous component is rarely present in carcinomas of certain sites, including the lung (1), breast (2), esophagus (3), stomach (4,5), colon (6,7) and urinary system (8). Furthermore, uterine corpus carcinomas with choriocarcinomatous differentiation have rarely been reported (917).…”
