Chondroprotective effects of a polycarbonate-urethane meniscal implant: histopathological results in a sheep model

Abstract: current findings provide preliminary evidence for the ability of an artificial PCU meniscal implant to delay or prevent osteoarthritic changes in knee joint following complete medial meniscectomy.

“…The anatomy of the ovine stifle joint has been well described and is considered a reasonable model of the human knee joint [20,21]. It has been used to investigate articular cartilage repair as well as anterior cruciate ligament (ACL) reconstruction [22] and meniscal repair [23] and replacement [24]. Caplan et al (2011) proposed the idea that MSCs work using a paracrine effect, essentially acting as a 'drug store' to aid in tissue regeneration [25].…”

“…The computed contact mechanics under the ultimate implant resembled those of an intact human meniscus measured from cadaveric experiments [51,52]. Subsequent in vivo experiments in a sheep model showed no differences in cartilage condition between the PCU implant and the contralateral control joints up to six months post-operatively [53]. However, Mankin scores of these non-operated controls were relatively high compared to earlier reported values [54], and it would be interesting to see if the chondroprotective effect is prolonged upon longterm implantation.…”

“…Publications on the design and testing processes of the disc-shaped, free-floating NUsurface® implant all originate from recent years [51][52][53]. The short span of time of these publications and the fact that the implant has already proceeded to phase 1 clinical trials illustrate the high speed at which progress can be made with synthetic implants that do not require tissue regeneration.…”
