Cholemic Nephropathy Reloaded

Abstract: Acute kidney injury (AKI) is a dreaded complication in patients with liver disease and jaundice, since it is associated with significant morbidity and mortality. Cholemic nephropathy (CN) is thought to represent a widely underestimated important cause of AKI in advanced liver diseases with jaundice. The umbrella term CN describes impaired renal function along with histomorphological changes consisting of intratubular cast formation and tubular epithelial cell injury directed primarily toward distal nephron seg… Show more

“…The most studied is neutrophil gelatinase-associated lipocalin (NGAL), a 25 kDa iron-transporting protein excreted in nephrotoxic or ischemic kidney injury. Urinary(u) NGAL levels in CN were shown to be suitable to monitor tubular epithelial damage and therapeutic effects under experimental conditions [ 1 , 45 ]. The diagnostic odds ratio (DOR) and sample size-weighted area under the curve for the receiver-operating characteristic (AUC-ROC) for NGAL to predict AKI were 18.6 (95% CI: 9.0-38.1)/0.815 (95% CI: 0.732-0.892) in a meta-analysis study of 19 studies with AKI [ 46 ].…”

“…The tubular injury characterizes it in the distal nephron segments secondary to the intratubular cast formation which causes obstruction and dilatation of the tubules. [81][82][83][84][85][86][87][88][89] In a large retrospective cohort study of postmortem kidney biopsies of 127 patients, bile cast nephropathy or cholemic nephrosis was observed in 72.1% of patients with ACLF versus 27.4% of patients with decompensated cirrhosis. The presence of high serum bilirubin and ACLF was significant predictors of cholemic nephrosis.…”

“…Hydrophobic bile acids are the most suspicious compounds in the pathogenesis of cholestasis-induced renal injury [ 4 , 5 , 8 , 9 ]. It has been well documented that hydrophobic bile acids can disrupt the function of different cellular targets, including biomembrane lipids, various proteins, as well as vital organelles such as mitochondria [ 8 , 10 , 11 ].…”
