Despite advent of CT and ultrasound, endoscopic retrograde cholangiopancreatography (ERCP) remains useful in the preoperative evaluation of the pancreas. Using radiographic criteria previously described, ERCP was accurate in 90% of 71 patients in differentiating benign from malignant disease. Diagnostic errors were caused by chronic pancreatitis mimicking carcinoma (N= 4) and carcinoma developing in a gland with chronic pancreatitis (N= 3) missed on ERCP. Chronic pancreatitis was associated with a normal ERCP in 10% (N=4) of patients.

Endoscopically obtained fluid from the ampulla of Vater was subjected to cytologic examination in 141 of 159 patients with suspected pancreatic disease, most of whom were undergoing endoscopic retrograde cholangiopancreatography (ERCP) examination. Of 26 patients with proven carcinoma of the pancreas, cannulation of the ampulla was successful in 22, and in 12 of these the cytology was positive (54%). Several technical considerations are suggested to increase the yield of positive cytology. They include aspiration of 3 ml or greater, deep insertion of the cannula, collection of pure pancreatic juice, high suction, and use of a fine intraductal rasp. The diagnostic sensitivity of ERCP alone was 73% in patients with cancer, but in combination with cytology it increased to 85%. The combined evaluation yielded 100% accuracy in 22 cases of cancer in which the ampulla was successfully cannulated. In 51 patients with chronic pancreatitis, and in 75 without proven pancreatic disease, the specificity of cytology was 98% and 97% respectively. The specificity of ERCP in chronic pancreatitis was 94%. Cytology is a useful adjunct to the ERCP examination in the diagnosis of pancreatic carcinoma.

Diagnostic accuracy of the cytologic examination or CEA measurement in pancreatic fluid obtained by percutaneous fine‐needle aspiration biopsy under ultrasonic guidance, was investigated in 26 patients with histologically proven carcinoma of the pancreas, and the incidence of positive results of cytology and CEA assay were compared in pancreatic fluid obtained by percutaneous fine‐needle aspiration biopsy and by endoscopic cannulation of the ampulla of Vater in the same 19 patients. Positive cytologic findings were observed in the fluid obtained by percutaneous aspiration biopsy of 88.5% of the patients with pancreatic carcinoma. The location of the cancer had no influence on the cytology, but positive results were more frequent in patients with distant metastases than in those with localized tumor or localy invasive carcinoma. Positive cytologic results were obtained in the pancreatic fluid obtained by endoscopic cannulation from the ampulla of Vater of only 15.8% of the patients examined by both methods, but percutaneous fine‐needle aspiration biopsy significantly increased the diagnostic rate. In the specimens obtained by aspiration biopsy, tumor cells were much more abundant and easily recognizable. When cytologic examination does not provide any evidence of malignancy, measurement of CEA levels in pancreatic fluid is probably useful. Combination of the cytology and CEA assay of the specimens obtained by percutaneous fine‐needle aspiration biopsy of the pancreas increased the diagnostic rate to 100%.
