Choices of therapy for exercise-induced asthma in children

Abstract: Cough and wheezing interferes with sport and other forms of physical activity in half of asthmatic children. Airway obstruction can be induced by a standard exercise test in over 70% of children with asthma. A beta-adrenergic agonist or cromone taken by inhalation beforehand will usually inhibit bronchoconstriction provoked by a free running exercise test. The duration of protective effect with salbutamol, terbutaline and cromones is less than 4 h. The long acting beta-adrenergic agonists formoterol and salmet… Show more

“…It is well known that oral administration of a soluble protein antigen induces a marked reduction in T-cell proliferative responses and antibody responses after a systemic challenge [8, 9]. However, in this study, oral administration of OVA-liposome conjugates did not affect T-cell cytokine production, nor did it inhibit anti-OVA IgG antibody production induced by immunization with OVA-alum.…”

“…The mucosal immune apparatus may thus provide an alternative and also a less expensive approach to vaccination. However, numerous attempts to administer foreign substances to mucosal membranes, predominantly by the oral route, have resulted in the induction of immune unresponsiveness due to what is referred to as oral tolerance [8, 9]. …”

“…Children with exercise-induced asthma do not show a long-term increase in airway reactivity [35]. It seems logical to provide prophylactic treatment that will facilitate participation in normal childhood games and activities.…”

“…According to the ATS guidelines, EIB is defined by a drop of ≥10% in Forced Expiratory Volume in one second (FEV 1 ) after exertion on an exercise test 6. The prevalence of EIB in children with asthma ranges from 40% to 90% 4 7 8. There is an ongoing debate about the mechanism whereby physical exercise can provoke EIB in asthma.…”
