Choanal Atresia: Birth Without Breath

Abstract: Bilateral Choanal atresia is a medical emergency. Corrective surgery is the mainstay of the treatment. Hegar's dilator was used in all cases to break the bony/membranous atretic plate. The 22 cases of choanal atresia all operated by the first author were included in this study. Eight cases were 1-5 year old with bilateral choanal atresia and all required immediate surgery as they had repeated attacks of respiratory distress and cycle of cyanosis. Eight cases were of CHARGE Syndrome. All the cases were operated… Show more

“…Infants are obligate nasal breathers because the oral airway is invariably blocked during calm respiration [ 11 ]. Infants breathe through the mouth only during crying, and infants have only nasal breathing until 5 months of life [ 11 ]. A one-third reduction in the diameter of the nasal airway can increase nasal airway resistance 81-fold, resulting in severe distress in neonates [ 11 ].…”

“…The results are hardly comparable, given the several surgical approaches included in the series [13,14,24]. More recent studies have described cases treated only with the endoscopic technique [15][16][17][18]. Schraff et al first compared postoperative outcomes in 14 CHARGE patients (nine with bilateral CA and five with unilateral CA), treated via transnasal (10 cases) or transpalatal (four cases) approaches.…”

“…In their experience, CHARGE association had a very unsolicited outcome. All CHARGE patients (eight cases) died 5-10 days after surgery due to complications of the syndrome [15]. A recent Italian multicentric study included 84 patients with CA.…”
