Aim:To investigate whether the use of an oxidizing mouth rinse as an adjunct to chlorhexidine is efficacious in reducing stains and plaque.Materials and Methods:This study had a single-blind, three-group (n = 35 each) parallel design, including a 21 days experimental period during which group I rinsed with chlorhexidine (CHX) 0.2% alone, group II used chlorhexidine (CHX) followed by hydrogen peroxide (H2O2) 1.5%. Group III rinsed with the same mouthwashes in reverse order. Patients were randomly assigned to one of the three groups. The examination for plaque, and stains was done after 1, 2, and 3 weeks of rinsing.Results:Group II showed significantly less stain intensity in comparison with group I after 14 and 21 days (P values 0.025 and 0.005, respectively). The proportion of stained surfaces was less in the group II than in the group I and was significant at the end of 1 week. The plaque formation was significantly less in groups II and III than group I at 7, 14, and 21 days.Conclusion:The adjunctive use of hydrogen peroxide to chlorhexidine proved to be superior to chlorhexidine alone with regard to the inhibition of plaque and development of stains.

This review demonstrates that when CHX mouthwash is recommended, it can be used in combination with an SLS dentifrice without any interference regarding its inhibiting effect on dental plaque, regardless of the order of use. Consequently, the collective evidence indicates that the combined use of dentifrice and CHX mouthwash is not contraindicated. However, this recommendation has been graded as moderate taking into account a potential publication bias because three of the four included studies emerged from the same research group.

Einleitung Die Karies ist bei Kindern und Jugendlichen inDeutschland seit über 2 Jahrzehnten in einem deutlichen Rückgang begriffen (Abb. 1) [1, 2]. So weisen die 12-Jährigen im Durchschnitt lediglich 0,7 bleibende Zähne mit (das Dentin einbeziehender) Karieserfahrung auf, und bei den 15-jährigen Deutschen sind im Mittel nur 1,8 Zähne mit Karies auszumachen. Damit zählen die deutschen Kinder und Jugendlichen, das bleibende Gebiss betreffend, heute international zu den Gruppen mit der geringsten Karieserfahrung. Dieser Erfolg ist auf eine Reihe von unterschiedlichen Maß-nahmen der Kariesprävention zurückzuführen. Ein Teil dieser Maßnahmen ist für alle Kinder und Jugendlichen gleichermaßen anzuwenden. Darüber hinausgehend sind jedoch andere Maßnahmen gezielt in Abhängig-keit vom jeweils individuellen Kariesrisiko indiziert. Im folgenden Beitrag soll die Anwendung der Maß-nahmen im Vordergrund stehen, ohne jedoch Aspekte der medizinischen Evidenz einzelner Maßnahmen außer Acht zu lassen. Kariesätiologie Im Mittelpunkt der Kariesätiologie steht die stoffwechselaktive bakterielle Plaque, in der bei zuckerhaltiger Ernährung Säuren entstehen, welche die Zahnoberflächen demineralisieren. Karies ist das Resultat dieser demineralisierenden Einwirkungen und gleichzeitig remineralisierender Vorgänge an der Zahnoberfläche, welche parallel zueinander auftreten.
