BIOLOGIAChlamydia trachomatis (C.t.) è un parassita endocellulare obbligato che appartiene alla famiglia delle Chlamydiaceae comprendente due generi, Chlamydia e Chlamydophila, e nove specie. Di queste solo tre sono responsabili di infezioni nell'uomo: C. trachomatis, appartenente al genere Chlamydia e C. pneumoniae e C. psittaci appartenenti al genere Chlamydophila (10). Mentre le infezioni da C. pneumoniae e C. psittaci interessano prevalentemente l'apparato respiratorio, C.t. ha un particolare tropismo per l'epitelio genitale e congiuntivale. Sulla base delle differenze antigeniche, sono stati identificati diciannove serovars di C.t.: i serovars A, B, Ba e C sono responsabili di patologie a carico dell'occhio e sono associati al tracoma endemico, i serovars D, Da, E, F, G, Ga, H, I, Ia e K sono più comunemente associati ad infezioni del tratto genitale a trasmissione sessuale, alle congiuntiviti da inclusi o alle polmoniti neonatali, infine i serovars L1, L2, L2a e L3 sono associati al linfogranuloma venereo (14).Il parassitismo endocellulare obbligato è spiegato dal fatto che C.t., incapace di sintetizzare ATP, deve necessariamente utilizzare quello della cellula ospite. Bacterial and protozoal sexually transmitted infections (STIs), such as Chlamydia trachomatis, Trichomonas vaginalis and Neisseria gonorrhoeae, may cause acute symptoms, chronic infections and severe long-term complications. The complications of these infections in women include pelvic inflammatory disease, chronic pelvic pain, tubal infertility, ectopic pregnancy, and infertility. Moreover, infection during pregnancy is associated with premature rupture of the membranes, low birth weight and miscarriage.In Italy, Chlamydia trachomatis and Trichomonas vaginalis infections are not subject to mandatory reporting; while gonorrhoea is subject to mandatory reporting.To extend surveillance to STIs that are widespread yet often asymptomatic and to improve the knowledge on the epidemiology of these infections in Italy, in 2009 the "Centro Operativo AIDS of the Istituto Superiore di SanitÁ", in collaboration with the Association of Italian Clinical Microbiologists (AMCLI, Associazione Microbiologi Clinici Italiani), launched the sentinel STIs surveillance system based on a network of 13 clinical microbiology laboratories.The main objective of the surveillance was to assess the prevalence and risk factors associated with Chlamydia trachomatis, Trichomonas vaginalis and Neisseria gonorrhoea infections among individuals attending microbiology laboratories in Italy.
