Background: The need for an anti-osteoporotic agent is in high demand since osteoporosis contributes to high rates of disability or impairment (high osteoporotic fracture), morbidity and mortality. Hence, the present study is designed to evaluate the protective effects of vanillic acid (VA) against bilateral ovariectomy-induced osteoporosis in female Sprague-Dawley (SD) rats. Materials and Methods: Forty healthy female adult SD rats were separated in to four groups with sham-operated control with bilateral laprotomy (Sham; n = 10), bilateral overiectomy (OVX; n = 10) group, OVX rats were orallay administrated with 50 mg/kg b.wt of VA (OVX + 50 VA; n = 10) or 100 mg/kg b.wt of VA (OVX + 100 VA; n = 10) for 12 weeks (post-treatment) after 4 weeks of OVX. Results: A significant change in the body weight gain was noted in OVX group, while treatment with VA substantially reverted to normalcy. Meanwhile, the bone mineral density and content (BMD and BMC) were substantially improved on supplementation with VA. Also, the bone turnover markers like calcium (Ca), phosphorous (P), osteocalcin (OC), alkaline phosphatase (ALP) and deoxypyridinoline (DPD) and inflammatory markers (IL-1β, IL-6, and TNF-α) levels were markedly attenuated in VA-treated rats. Moreover, the biomechanical stability was greatly ameliorated with VA administration. Both the dose of VA showed potent anti-osteoporotic activity, but VA 100 mg showed highest protective effects as compared with 50 mg of VA. Conclusion: Based on the outcome, we concluded that VA 100 showed better anti-osteoporotic activity by improving BMD and BMC as well as biomechanical stability and therefore used as an alternative therapy for treating postmenopausal osteoporosis.

PurposeThe aim is to systematically assess the effectiveness and safety of Chinese herbal formula Erxian decoction (EXD) for treating osteoporosis.Materials and methodsSix databases were searched from inception through September 17, 2016, without language restriction. All randomized controlled trials of EXD for osteoporosis were included. One or more outcome measures including fracture, change in bone mineral density (BMD), pain symptom improvement, bone biochemical markers, quality of life, adverse event or adverse drug reaction were evaluated. Study selection, data extraction, quality assessment, and data analyses were conducted according to Cochrane standards.ResultsEight trials including 644 patients investigated the effects of EXD in the treatment of osteoporosis. The methodological quality of the included trials was generally low. The meta-analysis from two trials showed favorable effects of EXD in improving BMD of lumbar spine (mean difference [MD]: 0.05 [0.03, 0.06]; I2=0%; P<0.00001) and BMD of femoral great trochanter (MD: 0.06 [0.02, 0.10]; I2=59%; P=0.005) compared with caltrate tablets. The other meta-analysis from two trials showed beneficial effects of EXD plus caltrate tablets and calcitriol in improving BMD of femoral neck (MD: 0.04 [0.00, 0.09]; I2=56%; P=0.04), the level of calcium (MD: 0.20 [0.15, 0.24]; I2=0%; P<0.00001), and phosphorus (MD: −0.28 [−0.39, −0.17]; I2=68%; P<0.00001) compared with caltrate tablets and calcitriol alone. The adverse drug reactions of EXD were mainly slight gastrointestinal symptoms.ConclusionThe study provides suggestive evidence of the superiority of EXD monotherapy or combination therapy over basic supplements for treating osteoporosis. However, the evidence remains weak. More rigorously designed and measured, randomized double-blind, placebo-controlled trials with larger sample size are needed to verify the current conclusions.

AbstrakTujuan : Penelitian ini bertujuan untuk mengetahui efek pemberian ekstrak rumput fatimah (Labisia pumila) terhadap osteokalsin serum dan deoxypiridinolin urin pada tikus post ovariektomi. Metode : Penelitian ini merupakan penelitian eksperimental, dilakukan di laboratorium Farmakologi dan Fisiologi Fakultas Kedokteran Universitas Brawijaya malang. Menggunakan tikus ovariektomi. Penelitian ini dibagi menjadi 5 kelompok, yaitu 1 kelompok kontrol negatif, 1 kelompok kontrol positif dan 3 kelompok perlakuan pemberian ekstrak rumput fatimah. Pengukuran osteokalsin dan deoxypiridinolin menggunakan elisa kit. Data hasil penelitian dianalisis dengan teknik analisis data.Hasil : Terdapat pengaruh pemberian ekstrak rumput fatimah terhadap penurunan osteokalsin serum dan peningkatan deoxypiridinolin urin pada tikus post ovariektomi. Tidak terdapat hubungan yang signifikan antara ekspresi deoxypiridinolin dengan ekspresi osteokalsin, hal ini menunjukkan bahwa perubahan ekspresi deoxypiridinolin tidak diikuti perubahan pada ekspresi osteokalsin. Pemberian rumput fatimah mampu mempengaruhi perubahan ekspresi deoxypiridinolin sebesar 53,46% dan perubahan ekspresi osteokalsin sebesar 66,97%. Sisanya dijelaskan oleh faktor lain yang tidak terlibat dalam penelitian. Kesimpulan : Pemberian ekstrak rumput fatimah dapat menurunkan osteokalsin serum dan meningkatkan deoxypiridinolin urin pada tikus post ovariektomi.Kata Kunci : Rumput Fatimah, Osteokalsin, Deoxypiridinolin Abstract Objective: This study was intended to investigate the extracurricular effects of serum osteocalcin and urinary deoxypiridinoline in post ovariectomy. Method: This research is an experimental research, conducted in Pharmacology and Physiology laboratory of Faculty of Medicine Universitas Brawijaya malang. Using ovariectomy rats. The study was divided into 5 groups, ie 1 group of negative control, 1 group of positive control and 3 treatment groups from grass fatimah extract. Measurement of osteocalcin and deoxypiridinoline using elisa kit. The result data were analyzed by data analysis technique. Result: There was an effect of giving fatimah grass extract to decrease of serum osteocalcin and increasing urine deoxypiridinolin in post ovariectomy rats. There was no significant association between deoxypiridinolin expression with osteocalcin expression, it showed that deoxypiridinolin expression changes did not follow changes in osteocalcin expression. Grass giving fatimah can change. The rest is explained by other factors not involved in the research. Conclusions: Provision of grass fatimah extract may decrease serum osteocalcin and increase urine deoxypiridinoline in mice after ovariectomy.
