Children with Chest Pain Visiting the Emergency Department

Chest pain is a worrisome symptom that often causes parents to bring their child to emergency department(ED) for evaluation. In the majority of cases, the etiology of the chest pain is benign, but in one-fourth of the cases symptoms are distressing enough to cause children to miss school. The clinician's primary goal in ED evaluation of chest pain is to identify serious causes and rule out organic pathology. The diagnostic evaluation includes a thorough history and physical examination. Younger children are more likely to have a cardiorespiratory source for their chest pain, whereas an adolescent is more likely to have a psychogenic cause. Children having an organic cause of chest pain are more likely to have acute pain, sleep disturbance due to pain and associated fever or abnormal examination findings, whereas those with non-organic chest pain are more likely to have pain for a longer duration. Chest radiograph is required in some, especially in patients with history of trauma . In children, myocardial ischemia is rare, thus routine ECG is not required on every patient. However, both pericarditis and myocarditis can present with chest pain and fever. Musculoskeletal chest pain, such as caused by costochondritis and trauma, is generally reproducible on palpation and is exaggerated by physical activity or breathing. Pneumonia with or without pleural effusion, usually presents with fever and tachypnea; chest pain may be presenting symptom sometimes. In asthmatic children bronchospasm and persistent coughing can lead to excess use of chest wall muscles and chest pain. Patients' who report acute pain and subsequent respiratory distress should raise suspicion of a spontaneous pneumothorax or pneumomediastinum. ED management includes analgesics, specific treatment directed at underlying etiology and appropriate referral.

Epidemiological, Etiologic and Clinical Features of Cases with Chest Pain in the Pediatric Cardiology Outpatient Clinic

Amaç: Göğüs ağrısı (GA), çocuk ve adölesanlarda sık görülmesine ve hasta ile ailenin kaygı duymasına neden olmasına rağmen, çoğu olguda etiyolojide organik bir sebep bulunmamaktadır. Çalışmamızda, GA nedeniyle başvuran çocuklarda epidemiyolojik, klinik özelliklerin ve etiyolojik nedenlerin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntemler: GA sebebiyle çocuk kardiyoloji polikliniğine Ocak 2015-Mart 2017 tarihleri arasında başvuran olgular yaş, cinsiyet, başvuru dönemi gibi epidemiyolojik; ağrının süresi, şekli, yayılımı, eşlik eden semptomlar, fizik muayene ve laboratuvar bulguları gibi klinik özellikleri açısından retrospektif olarak incelendi. Bulgular: Göğüs ağrısı şikayeti ile başvuran 570 olgunun ortalama yaşı 11.6±3.5 yıldı. En az olgu 7 yaştan küçük grupta (n:48,%8), en fazla ise 7-12 yaş (n:270,%47) grubundaydı (p=0.01). Erkek olgular kızlardan daha fazla olmasına rağmen aralarındaki fark anlamlı değildi (p=0.8). GA'na yol açan en sık nedenler idiyopatik (n:328,%58) ve kas-iskelet sistemi (n:109,%19) olarak tespit edildi. Kardiyak nedenler (n:45,%8) literatüre kıyasla daha fazla olup en sık iki neden mitral kapak prolapsusu (MVP) (n:20,%44) ve romatizmal kardit (n:7,%15)'di. Sonuç: Çocuklarda GA'nın etiyolojik nedenleri arasında kardiyak kökenli GA %0-4 oranında görülmektedir. Çalışmanın yapıldığı merkeze göre etiyolojik nedenlerin sıklığı değişebilmektedir. Çalışmamızda kardiyojenik GA beklenenin iki katı yüksek sıklıkta tespit edilmiştir. Olguların farklı birimlerden ön değerlendirme sonrası yönlendirilmiş olması bu duruma sebep olabilir. Literatürde kardiyak GA nedenleri arasında romatizmal kardit oldukça nadir bildirilmiş ancak çalışmamızda ikinci en sık neden olarak tespit edilmiştir. Kardiyojenik nedenler arasında özellikle gelişmekte olan ülkelerde romatizmal kardit hatırlanmalıdır. GA'a sebep olabilecek psikojenik nedenlerin tespiti için hastalar daha ayrıntılı sorgulanmalıdır.

OBJECTIVES: Psychological factors may be the underlying causes in unexplained chest pain (UCP). Chest pain symptom may influence the emotional status and peer relationships of the children and adolescents negatively. However, the number of studies focussing on the aetiology and consequences of the adolescent UCP are still limited. The aim of this study is to investigate the relationships among psychological problems, attachment characteristics, and the UCP in a group of adolescents. METHODS: Seventy-three adolescents with UCP and seventy-one healthy adolescents were included in the study. The adolescents completed the short form of Inventory of Parent and Peer Attachment (s-IPPA), and Strengths and Difficulties Questionnaire (SDQ), while their parents completed the parental form of the SDQ. RESULTS: Contrary to expectations, there was no significant interaction between total parental attachment levels and UCP in the adolescents with UCP. There were significant correlations between the attachment problems and total difficulties score of SDQ. Binary logistic regression analysis revealed that higher emotional and conduct problems and lower prosocial characteristics predict the UCP in adolescents, significantly. CONCLUSIONS: The results suggested that emotional/behavioural problems and lower prosocial behaviour scores are associated with UCP. However, further studies are needed for better understanding about the relationships between the UCP and attachment quality. ARTICLE HISTORY

Abstract: The most common cause of chest pain prompting a child to visit the ED is idiopathic chest pain. Careful physical examination can reveal important clues and save much unnecessary examinations.
