Aim. The aim of the current study is to analyze the medical and social dimensions of acute alcohol poisoning in children with regard of improving prevention of addictive habitis and develop effective preventative strategies for reducing underage alcohol consumption. Methods.We have studied the patients at the age up to 18 years with acute alcohol poisoning hospitalized in the Children Toxicology Department of Emergency Hospital Pirogov, Sofia, Bulgaria, from January 1, 2007 to June 31, 2008. All of them were at teen years -between 12 and 17 years old. Data on children were retrived from hospital medical records (age, sex, level of consciousness, blood ethanol level). The initial blood ethanol level was measured on admission by thin-layer chromatography for each patient. We have used the inquiry method -specially created for the purpose of the survey questionnaire comprising 39 questions (location and the reason for drinking, type of alcoholic beverage, age at first drink, combination alcohol-illicit drug, type of family, education and employment of parents, frequency of alcohol consumption by parents, consecutiveness of the children in family, presence of siblings, presence of children in a single room, interests etc.). Psychiatric interview was used.Results. We have studied 137 children with acute alcohol poisoning. Average age of children intoxicated by alcohol was 14.91±1.45 years. 77 (56.2%) are boys and 60 (43.8%) girls. The results demonstrate tendency of increase of the poisonings in weekends and in late afternoon and evening. On admission to hospital most of them had different levels of depressed consciousness: 61.3% were somnolent, 28.5% were soporous and 5.1% were comatose. Blood ethanol level was over 2.00 mg/ml in 40.2%. In the 21 (15.3%) cases children have been poisoned at the first alcohol consumption. No repeated hospitalization for acute alcohol poisoning in the study group for that period has been registered. Combination alcohol-illicit drug was observed in 13 children (9.5%). Children most often used one type of alcoholic beverage. The most frequent alcoholic beverage leading to intoxication is the vodka (63.1%). 64% of the children come from complete families. Both parents have secondary education in 79.7% and in 53.3% both parents are employed. 60% were the first born child in the family. First alcohol consuption was at the12 years and 10 months for boys, and at 13 years for girls. The most frequent reason for alcohol consumption was meeting and communicing with friends. The students have approximately 1.20 euro daily. Conclusion.Influence of "the group" is the main cause of alcohol consumption in adolescence. There is no increasing tendency for combining alcohol with other psychoactive drugs. We consider the brief intervention prior to hospital discharge as a preventive factor for repeated poisonings.

SUMMARYObjective: The aim of our study was to identify and analyze selected factors influencing alcohol use among primary school children aged 8 to15 years in the academic year 2009/10.Methods: The sample consisted of children from 28 primary schools. Data of this cross-sectional study were collected using questionnaires. Chi-Square test was used to test differences in proportions of observed phenomena between boys and girls. Multiple logistic regression analysis was performed to identify the influence of selected factors on the variable "child's alcohol drinking per week".Results: The sample consisted of 2,494 respondents (52% of boys, 48% of girls). In the study group 78% of all respondents (95% CI=76−80) drank alcohol infrequently (less than once a week) or did not drink alcohol at all, and 22% of respondents (95% CI=20−24) drank alcohol at least once a week.More boys than girls considered alcoholic beverages such as beer, wine and spirit as quite easily available. We performed the model of multivariate logistic regression analysis using the independent variables: age of respondents, gender of respondents, mother's alcohol drinking, father's alcohol drinking, sibling's alcohol drinking, parental rules, parental control, and mental support from parents (if their children have problems) to identify their effect on the dependent variable − child's alcohol drinking per week. We found out that mother's alcohol drinking and a lack of mental support from parents did not have a statistically significant influence on child's alcohol drinking per week. Conclusion:The results of our study point to the fact that it is necessary to focus on the prevention of alcohol drinking in general and among school children in particular. We recommend greater control of the sale of alcoholic beverages, so as to prevent the purchase by people under 18 years of age.

B Bu ul lg gu ul la ar r: : Çalışma süresinde çocuk acil ünitemize 32'si (%74,4) erkek ve 11'i (%25,6) kız olmak üzere toplam 43 çocuk olgu başvurmuştu. Hastaların yaş ortalaması 13,7±5,3 (1,0-17,0 yıl) idi. Zehirlenmelerin 33'ü (%76,7) aşırı tüketime, 7'si (%16,3) kaza sonucu ve 3'ü (%7,0) ise özkıyım amaçlı alıma bağlı gelişmişti. Kan alkol düzeyinin ortanca değeri 127 (1-314) mg/dL idi. Zehirlenme şiddet skoru (ZŞS) uyarınca 23 (%53,5) olguda zehirlenme hafif, 2 (%4,7) olguda orta ve 3 (%7,0) olguda ise ağır şiddette idi. Üç (%7,0) olgunun Glaskow koma skoru (GKS) ≤8 idi. En sık belirti ve bulgular sırasıyla bulantı (%48,8), kusma (%46,5), bilinç kaybı (%7,0) ve hipotermi (%2,3) idi. Üç (%7,0) olguda travma öyküsü, 1 (%2,3) olguda ise cinsel istismar bulgusu mevcuttu. İki (%4,7) olguda alkolle birlikte uyuşturucu madde kullanımı tespit edildi. Olguların ortalama takip süresi 16,1±7,1 (2,0-31,0) saatti. Kırk iki (%97,6) olguya damar içi sıvı ve 1 (%2,3) olguya damar içi sodyum bikarbonat tedavisi uygulanmıştı. S So on nu uç ç: : Çocuk acil servislerine alkol zehirlenmesi nedeniyle yapılan başvurular giderek artmakta olup, travma, uyuşturucu ilaç kullanımı ve psikiyatrik hastalıklar bu klinik tabloya eşlik edebilmektedir.
