We analysed the effects of number of siblings on the risk of solid tumours using the Swedish Family-Cancer Database, including population-based information on over 11 million individuals and more than 178 000 cancer patients diagnosed between 1958 and 2004. Incidence rate ratios (RRs), estimated by Poisson regression models, were adjusted for age, sex, birth cohort, area of residence and socioeconomic status. Having eight or more siblings vs none increased the risk of stomach cancer (RR ¼ 1.83, 95% confidence interval (CI), 1.44 -2.34). Anal cancer diagnosed before age 40 showed the strongest association with the total siblings (RR ¼ 3.27, 95% CI, 2.04 -5.26 for five or more siblings vs none). Endometrial (RR ¼ 0.76, 95% CI, 0.70 -0.82), testicular (RR ¼ 0.71, 95% CI, 0.62 -0.82), skin cancer (RR ¼ 0.82, 95% CI, 0.69 -0.97) and melanoma (RR ¼ 0.72, 95% CI, 0.65 -0.79) showed strong decreased risks for five or more siblings vs none. Prostate cancer risk for those with five or more older siblings vs none was 1.38 (95% CI, 1.23 -1.55). Having five or more younger siblings was most strongly associated with stomach cancer (RR ¼ 1.59, 95% CI, 1.29 -1.95) and melanoma (RR ¼ 0.68, 95% CI, 0.59 -0.79). We conclude that sibship characteristics are strong correlates of cancer risk at several sites; plausible interpretations include socioeconomic status.

Descritores ResumoRealizou-se revisão da epidemiologia da infecção pelo Helicobacter pylori, com o objetivo de analisar sua importância na infância e adolescência no que se refere à incidência, à prevalência, às vias de transmissão e aos fatores de risco. Para tanto, realizou-se uma pesquisa nas bases de dados Lilacs (OPS/Bireme) e Medline (National Library of Medicine) e no banco de teses da Universidade de São Paulo dos anos de 1983 a 1999. A análise da literatura levou à conclusão de que a infecção pelo Helicobacter pylori é adquirida principalmente durante a infância, apresenta aumento da prevalência com a idade, os principais fatores de riscos estão relacionados ao baixo nível socioeconômico e o mecanismo de transmissão permanece desconhecido. AbstractThe scope of the review is to study the epidemiological aspects of Helicobacter pylori infection and its importance during childhood and adolescence, focusing on incidence, prevalence, transmission and risk factors. The study's references included the following databases: LILACS (PAHO/ Bireme), MEDLINE, the US's National Library of Medicine and the thesis developed at University of São Paulo for the period 1983 to 1999. It was noted that Helicobacter pylori infection is mainly acquired during childhood, age-related prevalence, main risk factors are associated to low socioeconomic status, and its transmission mechanism remains unclear.

Background-Previous studies demonstrated an association between antibodies to mycobacterial heat-shock protein 65 (mHSP65) and carotid artery thickening. We examined whether mHSP65 antibodies are associated with levels of coronary calcification that appear to reflect preclinical coronary artery disease (CAD). Methods and Results-Serum specimens from 201 healthy asymptomatic subjects (52% male; mean age, 56.6 years) undergoing electron-beam computed tomographic imaging were used to measure levels of mHSP65 and human HSP60 antibodies and antibodies to several infectious pathogens. We found that 84% of the study subjects had anti-mHSP65 IgG antibodies. Mean titers of mHSP65 antibodies were higher (1:394 versus 1:267, Pϭ0.012) in individuals with than in those without elevated levels of coronary calcium (calcium score Ն150). Increasing titers of mHSP65 antibodies were significantly associated, in a dose-response manner, with elevated levels of coronary calcification. Individuals with the highest titers of mHSP65 antibodies (Ն1:800) had an adjusted odds ratio (OR) of 14.3 for having elevated coronary calcium (Pϭ0.004). Association of mHSP65 antibodies with elevated coronary calcification levels was independent of CAD risk factors after multivariate adjustment (Pϭ0.037). Interestingly, mHSP65 antibody titers were correlated with Helicobacter pylori infection (Pϭ0.004), which maintained significance after adjustment for CAD risk factors and seropositivities to other pathogens (adjusted OR, 3.1; 95% CI, 1.4 to 6.6). No association was found between antibodies to human HSP60 and levels of coronary calcification. Conclusions-Antibodies to mHSP65 are associated with elevated levels of coronary calcification and correlated with H pylori infection, suggesting that pathogen-triggered autoimmunity plays a role in early atherosclerosis.
