Experiences of childhood adversity are common and have profound health impacts over the life course. Yet, studying health outcomes associated with childhood adversity is challenging due to a lack of conceptual clarity of childhood adversity, scarce prospective data, and selection bias. Using a 65-year follow-up of a Swedish cohort born in 1953 (n = 14,004), this study examined the relationship between childhood adversity (ages 0–18) and premature all-cause mortality (ages 19–65). Childhood adversity was operationalized as involvement with child welfare services, household dysfunction, and disadvantageous family socioeconomic conditions. Survival models were used to estimate how much of the association between child welfare service involvement and mortality could be explained by household dysfunction and socioeconomic conditions. Results show that individuals who were involved with child welfare services had higher hazards of dying prematurely than their majority population peers. These risks followed a gradient, ranging from a hazard ratio of 3.08 (95% CI: 2.68–3.53) among those placed in out-of-home care, followed by individuals subjected to in-home services who demonstrated a hazard ratio of 2.53 (95% CI: 1.93–3.32), to a hazard ratio of 1.81 among those investigated and not substantiated (95% CI: 1.55–2.12). Associations between involvement with child welfare services and premature all-cause mortality were robust to adjustment for household dysfunction and disadvantageous family socioeconomic conditions. Neither household dysfunction nor socioeconomic conditions were related with mortality independent of child welfare services involvement. This study suggests that involvement with child welfare services is a viable proxy for exposure to childhood adversity and avoids pitfalls of self-reported or retrospective measures.

Mielenterveystyön hoidon alkuarvioinnissa selvitetään asiakkaan vahvuudet ja haasteet eri elämän osa-alueilla, punnitaan interventioiden tarve, hoidon tavoitteet ja hoidon seuranta. Mielenterveys- hoidon alkuarvioinnin tulee olla suunnitelmallista ja laadukasta, jotta potilaan oikeus hyvään hoitoon toteutuu. Tässä tutkimuksessa tarkasteltiin, millaisissa tulkintakehyksissä (Goffman 1986) hoidon alkuarviointi jäsentyy mielenterveystyön moniammatillisen hoitotiimin ja asiakkaan välillä. Tutkimusaineistoina ovat videoidut ja litteroidut kolme hoidontarpeen arviointikäyntiä ja asiakastapaamisten jälkeen työntekijöiden käymät reflektiokeskustelut psykiatrisella polikilinikalla. Aineiston analyysin tuloksena alkuarvioinnista identifioitiin hoidontarpeen, mielenterveyden, huolen, ja tulevaisuuden tulkintakehykset. Tulosten perusteella asiakkaan ja työntekijöiden toiminta ja roolit vaihtelivat kehyksittäin. Hoidontarpeen arvioinnin ja mielenterveyden tulkintakehyksessä tavoitteina oli saada tietoa potilaan kokonaistilanteesta, oireista ja tehdä diagnoosi. Hoitotiimin jäsenet esittivät kysymyksiä, ja potilas oli selontekovelvollinen. Huolen tulkintakehyksessä tavoitteena oli lievittää potilaan diagnoosin aiheuttamaa stigmatisoitumista ja aktivoida potilasta rohkaisevilla, suostuttelevilla ja selitystä pyytävillä kysymyksillä. Potilaalla oli hoitoa tarvitsevan sairastuneen rooli. Tulevaisuuden tulkintakehyksessä tavoitteena oli potilaan toimijuuden tukeminen ja aktivointi sekä potilaan hoidon oikea-aikaisuuden turvaaminen. Potilaalla oli vastuullisen toimijan rooli. Tulokset tuovat näkyväksi hoidon alkuarvioinnin ja sen merkityksen omana työvaiheenaan mielenterveystyössä. Lisäksi tulokset auttavat tunnistamaan prosessin eri vaiheita, potilaan ja työntekijöiden rooleja ja kysymysten merkitystä. Tuloksia voidaan hyödyntää alkuarvioinnin kehittämisessä, sosiaali- ja terveydenhuollon ammattilaisten koulutuksessa ja käytännön potilastyössä.

The transformative reality of diverse Canadian families is outpacing national and provincial statutes and policies. Social workers in child welfare agencies are faced with the complex task of making decisions about families while working within the confines of national/provincial statutes and social policies, as well as within agency structures. They attempt to balance the rights of diverse Canadian families and still protect children at risk of harm with the principle of the ‘best interest of the child’. The purpose of this qualitative case study was to explore the construction of ‘family’ and decisions about family life in protection services from the perspective of professional social workers in the prairie region of Canada. Social workers from several urban communities were invited to participate in focus groups. During the focus group discussions, themes of social worker’s nuanced and somewhat fluid understandings of family did not always converge with current legal and professional notions of families. Study findings suggest that social workers’ construction of family and the decisions they make about family life involve three primary themes: ‘acceptance of diverse understandings of family’; ‘safety and the best interest of the child’, and ‘professional discretionary decisions’
