Child Neurology: A Case Series of Heterogeneous Neuropsychiatric Symptoms and Outcome in Very Early-Onset Narcolepsy Type 1

Abstract: Narcolepsy type 1 is a central disorder of hypersomnolence characterized by excessive daytime sleepiness, cataplexy (i.e., sudden loss of muscle tone during wakefulness triggered by emotions), and rapid eye movement sleep-related manifestations that can present with a peculiar phenotype when arising at a pediatric age. Several features of childhood-onset narcolepsy type 1 are also common in neuropsychiatric conditions; discrete neuropsychiatric comorbidity has also been demonstrated. Here we report on three chi… Show more

“…Various neuropsychiatric symptoms (mood disturbances, irritability) and co-morbidities (such as attention deficit/hyperactivity disorder, oppositional defiant disorder) as well other developmental delays and a transient movement disorder (with ‘active’ and ‘negative’ motor symptoms such as swaying and falls respectively, as was seen in our patient) can occur in a child with narcolepsy. 7 , 13 Therefore, it only seems logical that the earlier narcolepsy is identified and treated, the better the neuropsychiatric and neurodevelopmental outcomes will be. 7 …”

“…In addition, thyrotrophin-releasing hormone and the nicotine patch are both promising new treatments for narcolepsy. Future research is required to validate and establish future treatment options as effective therapeutic strategies for narcolepsy [ 179 , 180 , 181 , 182 , 183 , 184 , 185 , 186 ]. Table 1 contains a list of potential therapeutics that may help with narcolepsy symptoms.…”

“…However, the peculiar clinical features of paediatric patients possibly manifesting EDS with behavioural changes and cataplexy as a hypotonic condition also in the absence of clear emotional triggers, 1 coupled with the possible early onset of concurrent neuropsychiatric symptoms, 5 can make clinical suspect challenging, often leading to a delay in proper referral and adequate diagnostic workup. 6 …”
